Relato de caso clínico de paciente que apresentou estenose traqueal e traqueomalácia pós intubação orotraqueal por tempo prolongado. Paciente do sexo feminino, com 47 anos, histórico de asma grave desde a infância, compareceu ao ambulatório de pneumologia queixando-se de disfonia, dispneia intensa em repouso, dor torácica em aperto e sibilância, sem melhora ao uso de salbutamol. Tendo sido internada anteriormente em unidade de terapia intensiva devido à exacerbação de crise asmática, permanecendo intubada por período maior a 14 dias e posteriormente traqueostomizada, mantendo assim por um período de dois meses. Foi realizada broncoscopia, que evidenciou granuloma ao nível de traqueostomia em parede anterior, traqueomalácia e estenose traqueal com diminuição de 20% da luz traqueal. Sendo necessária a ressecção de uma porção acima de 5 cm, seria indicado o uso de um substituto artificial para substituir a porção removida da traqueia, o que pode ser desenvolvido por meio de técnicas de engenharia de tecidos e impressão 3D.
Relato de caso de um paciente que apresentou quadro de paresia de membro superior direito, evoluindo para os demais membros, necessitando de internação. Semanas antes havia se submetido a vacinação contra COVID-19, porém, após a internação, foi feito o teste rápido para COVID-19, com resultado positivo. A investigação diagnóstica por meio de eletroneuromiografia identificou quadro indicativo de neuropatia axonal motora aguda, que é uma variante axonal motora pura da Síndrome de Guillain-Barré (SGB). Há vários outros relatos na literatura associando essa síndrome com a infecção por COVID-19. Esse é um fato importante que precisa ser reconhecido e suspeitado pelos médicos, pois a SGB traz sintomas de paresia flácida grave, que pode levar à admissão do paciente em unidade de terapia intensiva (UTI), afetando inclusive o sistema respiratório, e precisa ser diferenciada tanto dos sintomas de COVID-19 como de uma possível fraqueza adquirida na UTI após os tratamentos contra a infecção viral.Seu tratamento com a administração de imunoglobulina intravenosa é eficaz e deve ser iniciado assim que houver confirmação do diagnóstico, que pode ser feito através de eletroneuromiografia.

Relato de caso clínico de paciente que sofreu acidente vascular cerebral isquêmico (AVCi) após abuso do uso de Dietilamida do Ácido Lisérgico (LSD), apresentando síndrome da desinibição. Paciente masculino de 43 anos, sem comorbidades, mas com histórico de drogadição intensa, apresentou déficit neurológico súbito após abuso do uso de LSD. Evoluiu com cefaleia de forte intensidade, seguida de síncope. Ao despertar, apresentou desvio de rima labial para direita e hemiparesia esquerda. A tomografia computadorizada de crânio descartou sangramento, hipodensidade ou edema cerebral. Foi aventada hipótese de AVCi e realizada trombólise com Alteplase após 1 hora do início dos sintomas. O paciente foi transferido para a unidade de terapia intensiva, onde observou-se um quadro de sequela na região frontal, manifesta como síndrome da desinibição, estabelecendo contato verbal inadequado e depravado, com erotização de desinibição sexual. Familiares relataram que o paciente se encontrava com personalidade irreconhecível, em comparação a antes do evento cerebral. Síndromes comportamentais causadas por AVC são raras, assim, o acompanhamento desse caso foi relevante, pois além de não se encontrar na literatura outro caso de AVC causado por abuso de LSD, verificou-se o surgimento da síndrome da desinibição como manifestação sintomática.
Relato de caso de uma paciente do sexo feminino de 28 anos de idade, com antecedente de Diabetes mellitus tipo 1, a qual deu entrada em serviço de emergência em hospital regional em seu município de origem por um quadro respiratório de 08 dias de evolução manifestado por dispneia, náuseas, vômitos, dor abdominal e rebaixamento do nível de consciência. Após avaliação inicial foi diagnosticada com infecção viral aguda por SARS-COV-2 através de RT-PCR detectável e quadro concomitante de cetoacidose diabética através do quadro clínico e análise de gasometria arterial. Foi então transferida para Manaus e admitida em leito de Unidade de Terapia Intensiva em um Hospital e Pronto Socorro, recebendo alta após 22 dias de internação com melhora substancial do quadro respiratório e resolução do quadro de cetoacidose diabética. Ficou evidente que há uma correlação íntima entre infecção por SARS-CoV-2 e a ocorrência de Cetoacidose Diabética em pacientes diabéticos, além da necessidade de monitoramento e cuidados em UTI para o melhor desfecho.
