Pili torti also known as ‘twisted hairs’ (Latin:
pili=hair; torti=twisted) is a rare, congenital
or acquired clinical presentation, in which the hair shaft is flattened at irregular
intervals and twisted 180º along its axis. It is clinically characterized by fragile,
brittle, coarse and lusterless hairs, due to uneven light reflection on the twisted
hair surface. Pili torti may be associated with neurological
abnormalities and ectodermal dysplasias. There is no specifi c treatment for this
condition, but it may improve spontaneously after puberty. We report a case of
pili torti in a child who presented fragile, brittle, diffi cult
to comb hair. The patient had no comorbidities.
Por sua importância estética, os cabelos freqüentemente são alvo de interferências sociais, como é o caso de penteados e tinturas bizarras, e por vezes de auto-agressões. Os atos auto-agressivos mais freqüentes são: tricotilomania, tricotemnomania, tricofagia, tricoteiromania, pseudoalopecia da coçadura, tricocriptomania, tricorrexomania e plica neuropática. Neste artigo o autor discute detalhadamente as características clínicas e propedêuticas de cada doença, dando ênfase aos aspectos psiquiátricos dos pacientes.
A case of a patient with alopecia areata treated with diphencyprone, which produced contact dermatitis in the scalp, with later total regrowth is presented. After 4 months, patient presented a very intense hair loss compatible with telogen effluvium. The author calls attention for the possibility of telogen effluvium after contact dermatitis, once the production of the contact dermatitis is one of the choice treatments for alopecia areata.
Prurido braquiorradial é um tipo de prurido ou sensação de ardor intenso que acomete a região anterior do terço distal dos braços e terço proximal dos antebraços, correspondendo à região do músculo braquiorradial. A doença tem sido associada à radiação solar e também, cogitou-se, a lesões ortopédicas na coluna cervical. Uma das características da doença é a refratariedade ao tratamento. O autor apresenta duas pacientes com prurido braquiorradial tratadas com talidomida, com resultados excelentes.
Resumo: Coxins interfalangeanos são nodulações ceratósicas, de limites precisos, com aproximadamente um centímetro de diâmetro, geralmente sobre as articulações interfalangenas das mãos. A paquidermodactilia é uma tumefação uniforme da pele que ocorre nas falanges proximais das mãos. É relatado o caso de um jovem com associação de paquidermodactilia e coxins interfalangeanos, fato não encontrado na literatura médica. Embora haja descrição que considera serem essas manifestações diversas da mesma doença, acredita-se que sejam entidades distintas. O ato compulsivo de atritar a pele parece ser o denominador comum mais aceito para justificar as duas doenças. Os autores postulam que o coxim interfalangeano seria uma resposta epidérmica, enquanto a paquidermodactilia, uma resposta dérmica a um mesmo fator traumático sobre a pele. O paciente foi tratado com infiltração intralesional de triancinolona, com melhora clínica expressiva das duas manifestações. Palavras-chave: dedos; fibrose.
Summary: Knuckle pads are keratotic nodulations within precise limits and approximately one centimeter in diameter, usually on the interphalangeal joints of the hands. Pachydermodactyly is a uniform swelling of the skin occurring in the proximal phalanges of the hands. A case involving a young man suffering from several knuckle pad lesions concomitant with pachydermodactyly was studied. This association has not been previously described in the literature.
