INTRODUCCIÓNLa arteritis de Takayasu 1 (AT) es una inflamación granulomatosa de la aorta y sus principales ramas, además afecta principalmente a mujeres. Si bien la mortalidad es baja y la supervivencia durante 10 años es elevada, la evolución puede ser fulminante principalmente en niños, ya que en este grupo alcanza 33%, pero en adultos es extraordinaria. Debido a que presenciamos un caso excepcional en un adulto, en el que la evolución fue fulminante, efectuamos este trabajo.
Introducción: El síndrome autoinmune inflamatorio inducido por adyuvantes es una entidad descrita en 2011. Sus manifestaciones incluyen mialgias, artralgias, fatiga crónica y boca seca, así como manifestaciones neurológicas: alteraciones cognitivas, pérdida de memoria y discapacidades neurológicas. Caso clínico: Mujer de 40 años, con prótesis mamarias de 20 años, que en los últimos seis meses ha presentado: mialgias, fatiga, sensación de rigidez articular y artralgias generalizadas, incluyendo columna cervical y lumbar, dificultad para concentrase en su trabajo y alteraciones del sueño, cambios en estado anímico con irritabilidad y disnea al subir escaleras, incrementándose la fatiga principalmente en extremidades pélvicas. Los estudios de laboratorio relevantes mostraron insuficiencia de vitamina D, elevación de proteína C reactiva y velocidad de sedimentación globular (VSG) incrementada; la mamografía reportó contracturas tipo II de Baker. Se efectuó retiro de prótesis mamarias y prescripción de vitamina D, a seis meses del postoperatorio, paciente asintomática efectuando ejercicio aeróbico cinco días de la semana. Conclusión: Los implantes de silicón pueden llegar a producir el síndrome de ASIA en pacientes inmunológicamente susceptibles.
La arteritis de Takayasu es una enfermedad inflamatoria de las arterias de mediano y gran calibre con predilección por la aorta y sus ramas. Las lesiones producidas pueden ser estenóticas, oclusivas o aneurismáticas. Los cambios vasculares conducen a las principales complicaciones. Aunque el manejo de la arteritis de Takayasu está mejorando, el diagnóstico generalmente se retrasa. Si bien el uso más amplio de imágenes no invasivas ha mejorado, los médicos seguimos teniendo la responsabilidad de considerar el diagnóstico de manera más temprana. Se efectúa una revisión de la situación actual. La comprensión de la patogenia de la enfermedad es escasa; los datos genéticos y la identificación de citocinas patógenas pueden facilitar la búsqueda de biomarcadores capaces de distinguir entre enfermedad activa e inactiva, remodelación arterial inflamatoria y no inflamatoria. La imagenología es crucial para el diagnóstico y la evolución de la enfermedad. La dependencia de la terapia con corticosteroides sigue siendo alta. Sin embargo, actualmente se reconoce el impacto de la terapia inmunosupresora combinada, las terapias biológicas están cada vez más disponibles y los nuevos agentes son prometedores, tratando de modificar la morbilidad y la mortalidad.
Introduction: polymyositis, a relatively uncommon autoimmune disorder, develops due to abnormal activation of cytotoxic T lymphocytes and macrophages against muscle antigens presenting as a proximal myopathy is associated with interstitial lung disease in 10 to 30% of patients. Case report: 76-yearold male with a slow evolution of eight months, characterized by fatigue, weight loss of 12 kilos, and increasing weakness in cervical paravertebral muscles and muscles of the shoulder and pelvic girdles. Laboratory showed elevated muscle enzymes, positive Anti J-1 antibodies, consistent EMG data, and a biopsy confirming polymyositis, associated with interstitial lung disease. Steroid treatment and rehabilitation were started, obtaining significant clinical improvement at three weeks. Conclusion: since polymyositis is not frequent, the diagnosis is reached by exclusion mainly when the symptomatology and the tomographic images coincide during COVID-19.
Introducción: El síndrome de Guillain-Barré (SGB) generalmente se considera monofásico, pero las recurrencias ocurren en un subgrupo de pacientes con tasa de recurrencia de 1 a 6%. Con el advenimiento de la pandemia de SARS-CoV-2 los reportes del SGB se han incrementado, además de surgir SGB recurrente. Caso clínico: Masculino de 46 años, que seis años atrás presentó SGB asociado con Campylobacter jejuni, con recuperación ad integrum, y acude en esta ocasión con recurrencia del SGB, pero con diferencia de ser más severa la sintomatología y con carácter descendente, a diferencia del primer episodio. En ambas ocasiones fue atendido por el mismo equipo médico, manejado de manera similar, logrando recuperación más tardía en el segundo episodio. Conclusiones: Aunque aún existe controversia en la literatura, el SGB recurrente se presenta en personas más jóvenes y los cuadros pueden ser más severos en los episodios subsecuentes.
