Esta es una extraordinaria historia de la locura y de la psicopatología en la modernidad colombiana, elaborada con amplitud y profundidad. Como sedebate en estudios recientes, la “modernidad” colombiana, en muchos aspectos, es una periodización discutible. Sin embargo, esta obra designa con ello la entrada en Colombia, en la década de 1870, de discursos propiamente psiquiátricos, aunque precedan a la psiquiatría como especialidad universitaria. Se trató de discursos modernos porque concebían la locura como problema de la patología, la sometían a observación para analizarla y presuponían para ella una clínica, una nosología y una terapéutica. A finales de siglo XIX, además de asumirla como entidad nosológica, la locura comenzó a concebirse como constructo sociocultural enfrentado a la trasgresión de un orden “natural” y moral. Se volvió objeto de normalización, en tanto desbordaba el campo médico y comprometía la vida cotidiana. Por eso, el problema de estudio se aborda en el libro desde los ejes locura intramuros y locura extramuros.
Este artículo analiza algunas asociaciones médico-jurídicas entre locura y criminalidad en el departamento de Antioquia (Colombia), en las primeras tres décadas del siglo XX. El análisis se orientó por dos ejes imbricados: el de los discursos y el de las prácticas. Se examinaron las ideas de cuatro médicos, planteadas entre 1917 y 1925, para identificar los debates teóricos desde los cuales se delimitaban y definían las enfermedades mentales en casos judiciales. La puesta en escena del saber de los peritos y su lugar como expertos se analizaron en un caso judicial, que inició en 1921, y en cuyo desarrollo afloraron las confrontaciones teóricas entre los médicos que debatieron sobre la posible locura del acusado.
RESUMEN La silicosis es una enfermedad asociada al trabajo minero, cuya importancia se comenzó a reconocer en los campos médico, económico y jurídico desde 1930. Se ha convertido en un importante objeto de estudio para la historia de la medicina social y del trabajo. Su historia permite estudiar la adopción del concepto de "enfermedad profesional" y el nacimiento de la medicina del trabajo en Colombia. Este artículo analiza ese proceso a través de una lectura de las publicaciones médicas de la primera mitad del siglo XX sobre la silicosis en la minería colombiana.
A fines del siglo XIX, el comercio internacional y la navegación a vapor facilitaron la tercera pandemia de peste bubónica que comenzó en China en 1891 y llegó al continente americano en 1898. Este flagelo parece haber tocado las costas colombianas entre 1913 y 1915, en pleno auge de la medicina pasteriana. Las deficiencias del aparato científico-sanitario estatal colombiano, basado en la bacteriología y la epidemiología nacientes no le permitieron al Estado y al cuerpo médico responder al miedo y a los rumores de epidemia que lograron afectar negativamente el comercio. Tampoco pudieron las autoridades enfrentar el problema con medidas apropiadas para el diagnóstico, el tratamiento de pacientes, el saneamiento urbano y el aislamiento de los lugares contaminados. Estas dificultades provocaron una confrontación entre el Estado y el cuerpo médico, suscitando una discusión sobre la real existencia de la peste. Esta discusión fue zanjada por la medicina oficial norteamericana que, en su veredicto, dio preferencia a los intereses comerciales de Estados Unidos, ignorando las urgencias sanitarias de la costa atlántica colombiana.
La medicina antioqueña del siglo XIX surgió como una propuesta oficial, en un esquema de medicina urbana y basada en el modelo francés de los siglos XVIII y XIX. Este esquema impulsaba la intervención médica autoritaria en casi todos los dominios del ordenamiento urbano. A finales del siglo pasado aparecieron en Medellín las políticas sanitarias medicalizadoras administradas por una institución oficial centralista en la cual los médicos jugaron un papel muy importante como funcionarios. Varios médicos muy destacados, formados en Francia o inspirados en la medicina gala (pasterianos y prepasterianos), impulsaron en esta ciudad un tipo de higiene basado en la mezcla de varias teorías que explicaban la causa de las enfermedades epidémicas y endémicas. La época dorada de este movimiento la constituyó el último decenio del siglo, cuando se aceptaron plenamente en Medellín las teorías de Louis Pasteur. Así, la naciente Bacteriología tuvo un papel protagónico no sólo en el ejercicio médico sino en el ordenamiento urbano de la ciudad.
