A sarcoidose é uma doença sistêmica de etiologia desconhecida, caracterizada pela inflamação crônica granulomatosa, que acomete com maior frequência os pulmões, a pele e os olhos e, muito raramente, detectamos envolvimento renal na patologia. Relatamos aqui o caso de uma paciente de 47 anos, branca, internada em uma unidade de emergência de Hospital Universitário, devido a sintomas e achados radiológicos sugestivos de pneumonia. Desde o início da internação ela se apresentava com quadro laboratorial de insuficiência renal aguda, inicialmente atribuída ao histórico recente de uso abusivo de anti-inflamatórios. No entanto, devido à evolução arrastada e aparentemente desfavorável, inclusive com necessidade de terapia renal substitutiva (TRS), a paciente foi submetida a uma biópsia renal para esclarecimento diagnóstico e avaliação prognóstica. A descrição histológica mostrava um quadro de nefrite intersticial aguda granulomatosa, característico de sarcoidose renal, diagnóstico confirmado após revisão do prontuário médico, que revelava internação anterior devido a evento pulmonar da doença. Iniciado tratamento com prednisona 1 mg/kg/dia, a paciente evoluiu com melhora do quadro de insuficiência renal aguda, não mais necessitando de TRS. Atualmente, a paciente é mantida em acompanhamento ambulatorial com função renal estável.Palavras-chave: sarcoidose, insuficiência renal aguda, nefrite intersticial aguda, hipercalcemia. AbstRActSarcoidosis is a systemic disease of unknown etiology, characterized by chronic granulomatous inflammation, which affects most frequently the lungs, skin, and eyes, and most rarely the kidneys. We report the case of a 47-year-old, white, woman admitted to the emergency unit of a university-affiliated hospital due to symptoms and radiologic findings suggestive of pneumonia. Since the beginning, her laboratory tests showed acute renal failure, initially attributed to the recent history of abuse of anti-inflammatory drugs. However, because of her protracted and apparently unfavorable evolution, requiring renal replacement therapy (RRT), she underwent renal biopsy for diagnosis and prognostic assessment. Histology showed acute granulomatous interstitial nephritis, characteristic of renal sarcoidosis, which was confirmed after review of the medical record disclosing a previous admission due to lung involvement of the disease. Treatment with prednisone (1mg/ kg/day) was initiated, acute renal failure improved, and the patient no longer required RRT. Currently she is followed up on an outpatient service, and her renal function is stable.
The patient is a 47-year-old white woman who was on the hemodialysis from 1999 to 2002, when she received a cadaveric renal transplant (deceased donor). The immediate postoperative immunosupression consisted of oral tacrolimus, predinisone and mycofenolato mofetil (MMF), and her medications at that time of her admission were tacrolimus (5mg 12/12 h), MMF (1000mg 12-12 h), and predinisone (10mg/ day). After 8 weeks, the patient went to the hospital and she was admitted presenting fever (37,9ºC), cough, malaise and vomiting. The chest radiography revealed a mass in the left superior lobe, which was initially treated with levofloxacin associated to ceftriaxone. There was partial improvement of the cough and total remission of the fever. The patient was discharged after 3 weeks of treatment in stable condition, with negative blood and bronchial cultures. After ten days, she returned to the hospital with relapsed symptoms and a subcutaneous purulent collection was detected in her left leg, and the culture of the drained material evidenced a filamentous microorganism, identified as Nocardia sp, later specified as Nocardia asteroides. Treatment with sulfametoxazole-trimetroprin 800mg t.i.d was initiated, and after five days, the patient was pyretic and treatment was kept for six months.
Este relato descreve o caso de uma paciente de 44 anos branca, mantida em programa de hemodiálise de 2001 a 2005, quando então recebeu transplante renal de doador falecido, terapia imunossupressora inicial constando de Tacrolimus, Prednisona e Micofenolatomofetil, sendo que, no momento da internação, as doses utilizadas eram, respectivamente: 3mg 12/12h, 10mg/dia e 500mg 12/12h. Dois anos após o transplante, a paciente apresentou quadro de lesão papular no lábio superior à direita, que inicialmente apresentou cicatrização espontânea, sem que o diagnóstico tivesse sido estabelecido, não tendo sido avaliada pela equipe de transplante que a acompanhava. Posteriormente retornou com as mesmas lesões, agora com acometimento da asa nasal e lábio superior à esquerda, acompanhadas ainda de febre intermitente, tosse seca seguida de expectoração e disfonia. Foi internada para investigação etiológica, realizada biópsia das lesões nasal e transbrônquica que revelou presença de Histoplama capsulatun. Confirmado o diagnóstico, foi iniciado tratamento com Itraconazol 400mg/dia. No sétimo dia de internação, a paciente passou a apresentar quadro de vômitos incoercíveis, dor e distensão abdominal, parada da eliminação de fezes e flatus, oligúria e hipotensão arterial com piora da função renal, além de hipoalbuminemia e acidose metabólica. Tendo evoluído com severa instabilidade hemodinâmica e importante insuficiência respiratória, a paciente foi encaminhada à Unidade de Terapia Intensiva, evoluindo para óbito 24 horas depois. A histoplasmose é uma complicação importante no pós-transplante e deve ser sempre lembrada em nosso meio, por ser o Brasil uma área endêmica e pela trágica evolução que a patologia pode apresentar.
Apesar dos benefícios do uso de imunossupressores para prevenir perda do enxerto no transplante renal, eles apresentam vários efeitos colaterais, entre eles as doenças linfoproliferativas que levam, por vezes, à necessidade de redução ou até mesmo da retirada total das drogas imunossupressoras, com risco de rejeição ao enxerto. Relatamos um caso de um paciente branco, 20 anos, portador de insuficiência renal crônica secundária à Síndrome de Prune Belly, tendo iniciado hemodiálise em 2004, e, após seis meses, submetido a transplante renal e mantida imunossupressão com Tacrolimus, Micofenolato mofetil e Prednisona. No quarto mês pós-transplante renal desenvolveu quadro de doença linfoproliferativa pós-transplante, sendo então retirados imediatamente o Tacrolimus e Micofenolato mofetil, mantido apenas com Prednisona. Após aproximadamente um mês da retirada da imunossupressão, como não houve boa resposta com a redução da imunossupressão associada à terapia antiviral, optamos pela associação de Rituximab ao tratamento, com acompanhamento através de tomografia computadorizada, mostrando remissão total da esplenomegalia e da linfadenomegalia. Baseados em dados da literatura, foi associado Sirolimo à imunossupressão do paciente e mantida Prednisona com boa evolução após dois anos de seguimento.
