Ленинградская, 70 РЕЗЮМЕ Рак предстательной железы (РПЖ) является одной из наиболее актуальных проблем современной онкоурологии по причине сохраняющихся высоких показателей заболеваемости данной патологией. Основными методами радикального лечения боль-ных локализованным и местно-распространенным РПЖ являются радикальная простатэктомия (РПЭ) и лучевая терапия -ди-станционная (ДЛТ) или брахитерапия. Тем не менее частота биохимического маркерного прогрессирования заболевания по-сле проведения радикальных методов терапии остается высокой и достигает 27-53%. При этом крайне важным является поиск опухолевого очага, приводящего к повышению маркера. У больных с появлением отдаленных метастазов единственным обще-принятым методом до настоящего времени являлась паллиативная гормональная терапия (ГТ). Тем не менее, у значительного числа пациентов маркерный рецидив может быть ассоциирован с так называемым олиго-метастатическим прогрессированием заболевания, при котором количество выявленных метастатических очагов минимально. Как показывают результаты исследо-ваний, применение хирургического или лучевого методов лечения у отобранных пациентов данной когорты позволяет суще-ственно увеличить продолжительность времени до назначения ГТ, а в ряде случаев полностью отказаться от ее проведения.Цель исследования. Оценить результаты спасительной тазовой лимфаденэктомии у больных РПЖ с прогрессированием заболевания после радикальных методов терапии.Пациенты и методы. В статье обобщены результаты хирургического лечения больных, которым выполнена спасительная лимфаденэктомия (СЛАЭ) по поводу олиго-метастатического лимфогенного прогрессирования РПЖ после проведенного лечения в 3 центрах -МНИОИ им. П. А. Герцена, филиале НМИРЦ; НИИ онкологии им. Н. Н. Петрова и ФГБУ РНЦРХТ МЗ РФ. В данном многоцентровом исследовании оценены непосредственные и отдаленные результаты хирургического лечения 57 пациентов.Результаты. Результаты исследования показали высокую эффективность методики спасительной тазовой лимфаденэкто-мии у больных РПЖ с прогрессированием заболевания после радикального лечения. Так, ответ на проведенную терапию в виде снижения уровня маркера отмечен у 47 (82,5%) больных. Снижение уровня ПСА ≥50% от исходного значения через месяц после операции зарегистрировано у 38 (66,7%) больных. Полный ответ в виде снижения маркера ≤0,2 нг/мл отмечен у 13 (22,8%) больных. Background: Some reports have suggested that nephron-sparing surgery (NSS) may protect against cardiovascular events (CVe) when compared with radical nephrectomy (RN). However, previous studies did not adjust the results for potential selection bias secondary to baseline cardiovascular risk. Objective: To test the effect of treatment type (NSS vs RN) on the risk of developing CVe after accounting for individual cardiovascular risk. Design, setting, and participants: A multi-institutional collaboration including 1331 patients with a clinical T1a-T1b N0 M0 renal mass and normal renal function before surgery (defined as an estimated glomerular filtration rate 60 ml/min/1.73 m
surgeon, pathological T and N stage) and the presence of CSL.• Univariable and multivariable linear regression models were also used to test the association between the available predictors and lymphorrhoea. RESULTS• The median (range) number of LNs removed was 20 (1-63). Both linear and logistic multivariable regression analysis showed that the number of removed LNs and age were the only two statistically significant predictors of total amount of lymphorrhoea and CSL after RRP and PLND (both P < 0.01).• Specifically, the risk of developing a CSL increased by 5% for every LN removed. Similarly, every year of age increased the risk of having CSL by 5%.• The most informative thresholds for predicting CSL were 65 years of age and 20 LNs removed.• External iliac lymphadenectomy resulted in a higher associated risk of lymphorrhoea and CLS relative to obturator LN removal ( P = 0.001 vs P = 0.1, respectively). CONCLUSIONS• There was a positive association between the number of LNs removed and age at RRP with the amount of lymphorrhoea and the risk of developing a CSL.• The most informative thresholds in predicting CSL were 65 years of age and 20 LNs removed. Lymphorrhoea is the leakage of lymph out of drains after surgery and may affect the perioperative period after retropubic radical prostatectomy (RRP) and extended pelvic lymphadenectomy (PLND). Similarly, lymphoceles represent a common complication after RRP and PLND, although only in few cases do lymphoceles become symptomatic.In the current study, the amount of lymphorrhoea and the risk of CSL in patients treated with RRP and PLND were prospectively assessed. Moreover, individual factors which can predict lymphorrhoea, CSL and total days of drainage, were identified, in order to improve treatment decision-making in PCa patients. Objective• To test whether the number of lymph nodes removed affects cancer-specific survival (CSS) or metastatic progression-free survival (MPFS) in different renal cell carcinoma (RCC) scenarios. Methods• We used Cox regression analyses to analyse the effect of the number of lymph nodes removed on CSS and MPFS in 1983 patients with RCC treated with nephrectomy.• To adjust for possible clinical and surgical selection bias, analyses were further adjusted for number of positive nodes, presence of metastases, age, performance status, T stage, tumour size and grade. Results• The prevalence of lymph node invasion was 6.1%. The mean follow-up period was 83.3 months.• Multivariable analyses showed that the number of nodes removed had an independent, protective effect on CSS in patients with pT2a-pT2b or pT3c-pT4 RCC (hazard ratio [HR] 0.91, P = 0.008 and HR 0.89, P < 0.001, respectively), in patients with bulky tumours (tumour size >10 cm, HR 0.97, P = 0.03) or when sarcomatoid features were found (HR 0.81, P = 0.006).• The removal of each additional lymph node was associated with a 3-19% increase in CSS.• When considering MPFS as an endpoint, the number of nodes removed had an independent, protective effect in the same patient categories. Conclusions• When clinically indicated, the number of nodes removed affects CSS and MPFS in specific sub-categories of patients with RCC.
