In this medical-surgical ICU, an extended visitation model was associated with reduced occurrence of delirium and shorter length of delirium/coma and ICU stay.

IMPORTANCE The effects of intensive care unit (ICU) visiting hours remain uncertain. OBJECTIVE To determine whether a flexible family visitation policy in the ICU reduces the incidence of delirium. DESIGN, SETTING AND PARTICIPANTS Cluster-crossover randomized clinical trial involving patients, family members, and clinicians from 36 adult ICUs with restricted visiting hours (<4.5 hours per day) in Brazil. Participants were recruited from April 2017 to June 2018, with follow-up until July 2018. INTERVENTIONS Flexible visitation (up to 12 hours per day) supported by family education (n = 837 patients, 652 family members, and 435 clinicians) or usual restricted visitation (median, 1.5 hours per day; n = 848 patients, 643 family members, and 391 clinicians). Nineteen ICUs started with flexible visitation, and 17 started with restricted visitation. MAIN OUTCOMES AND MEASURES Primary outcome was incidence of delirium during ICU stay, assessed using the CAM-ICU. Secondary outcomes included ICU-acquired infections for patients; symptoms of anxiety and depression assessed using the HADS (range, 0 [best] to 21 [worst]) for family members; and burnout for ICU staff (Maslach Burnout Inventory). RESULTS Among 1685 patients, 1295 family members, and 826 clinicians enrolled, 1685 patients (100%) (mean age, 58.5 years; 47.2% women), 1060 family members (81.8%) (mean age, 45.2 years; 70.3% women), and 737 clinicians (89.2%) (mean age, 35.5 years; 72.9% women) completed the trial. The mean daily duration of visits was significantly higher with flexible visitation (4.8 vs 1.4 hours; adjusted difference, 3.4 hours [95% CI, 2.8 to 3.9]; P < .001). The incidence of delirium during ICU stay was not significantly different between flexible and restricted visitation (18.9% vs 20.1%; adjusted difference, −1.7% [95% CI, −6.1% to 2.7%]; P = .44). Among 9 prespecified secondary outcomes, 6 did not differ significantly between flexible and restricted visitation, including ICU-acquired infections (3.7% vs 4.5%; adjusted difference, −0.8% [95% CI, −2.1% to 1.0%]; P = .38) and staff burnout (22.0% vs 24.8%; adjusted difference, −3.8% [95% CI, −4.8% to 12.5%]; P = .36). For family members, median anxiety (6.0 vs 7.0; adjusted difference, −1.6 [95% CI, −2.3 to −0.9]; P < .001) and depression scores (4.0 vs 5.0; adjusted difference, −1.2 [95% CI, −2.0 to −0.4]; P = .003) were significantly better with flexible visitation. CONCLUSIONS AND RELEVANCE Among patients in the ICU, a flexible family visitation policy, vs standard restricted visiting hours, did not significantly reduce the incidence of delirium.

OBJECTIVE: to assess the incidence and risk factors associated with nasal septum injury in premature infants using reused and new nasal prongs. METHOD: the study was a cohort from an open therapeutic intervention. The sample included 70 infants with a gestational age inferior to 37 weeks, who used nasal prongs and were hospitalized at the neonatal service of a hospital in Recife-PE, in the Northeast of Brazil. The data were collected in patient files through the assessment of the application of the device and of the nasal septum. Multinomial Logistic Regression and Survival analyses were applied. RESULTS: the incidence of nasal injury corresponded to 62.9%. In the multiple analysis, only the length of the infant's treatment was a determinant factor for the occurrence and severity of the injuries. CONCLUSION: the type of nasal prong does not serve as a risk factor for the nasal injury. The high incidence of nasal injury indicates the need to adapt the nursing care with emphasis on prevention.

Objective To contribute to updating the recommendations for brain-dead potential organ donor management. Method A group of 27 experts, including intensivists, transplant coordinators, transplant surgeons, and epidemiologists, joined a task force formed by the General Coordination Office of the National Transplant System/Brazilian Ministry of Health (CGSNT-MS), the Brazilian Association of Intensive Care Medicine (AMIB), the Brazilian Association of Organ Transplantation (ABTO), and the Brazilian Research in Intensive Care Network (BRICNet). The questions were developed within the scope of the 2011 Brazilian Guidelines for Management of Adult Potential Multiple-Organ Deceased Donors. The topics were divided into mechanical ventilation, hemodynamic support, endocrine-metabolic management, infection, body temperature, blood transfusion, and use of checklists. The outcomes considered for decision-making were cardiac arrest, number of organs recovered or transplanted per donor, and graft function/survival. Rapid systematic reviews were conducted, and the quality of evidence of the recommendations was assessed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) system. Two expert panels were held in November 2016 and February 2017 to classify the recommendations. A systematic review update was performed in June 2020, and the recommendations were reviewed through a Delphi process with the panelists between June and July 2020. Results A total of 19 recommendations were drawn from the expert panel. Of these, 7 were classified as strong (lung-protective ventilation strategy, vasopressors and combining arginine vasopressin to control blood pressure, antidiuretic hormones to control polyuria, serum potassium and magnesium control, and antibiotic use), 11 as weak (alveolar recruitment maneuvers, low-dose dopamine, low-dose corticosteroids, thyroid hormones, glycemic and serum sodium control, nutritional support, body temperature control or hypothermia, red blood cell transfusion, and goal-directed protocols), and 1 was considered a good clinical practice (volemic expansion). Conclusion Despite the agreement among panel members on most recommendations, the grade of recommendation was mostly weak. The observed lack of robust evidence on the topic highlights the importance of the present guideline to improve the management of brain-dead potential organ donors.

ABSTRACT. Anthracnose caused by the fungus Colletotrichum lindemuthianum is one of the main diseases affecting the common bean (Phaseolus vulgaris L.), and the pathogen is characterized by wide variability, with more than 50 physiological races identified in Brazil. Greater occurrences of races 65, 73, and 81 have been observed in Brazil along with the occurrence of pathogenic variability among isolates of a single race, destabilizing the resistance of commercial cultivars. Therefore, the aim of this study was to identify physiological races of C. lindemuthianum isolates collected in the states of São Paulo and Santa Catarina, Brazil and to test for variability among the isolates of race 65. The classification of 51 isolates resulted in the identification of 10 different physiological races: 4, 38, 55, 65, 73, 81, 83, 85, 321, and 351. Races 65 and 81 predominated, with frequencies of 37.25 and 35.29%, respectively. Regarding the isolates of race 65, wide physiological variability was evident, suggesting that a new differential set should be applied to detect the levels of variation among isolates of a single race of the pathogen.Keywords: physiological variability, levels of variation, anthracnose. Classificação de raças de Colletotrichum lindemuthianum em cultivares diferenciadoras de feijoeiro comum RESUMO.A antracnose causada pelo fungo Colletotrichum lindemuthianum é uma das principais doenças do feijoeiro comum (Phaseolus vulgaris L.), o patógeno caracteriza-se por apresentar uma grande variabilidade, com mais de 50 raças fisiológicas identificadas no Brasil. Tem-se evidenciado a maior ocorrência das raças 65, 73 e 81 no país e também a ocorrência de variabilidade patogênica entre isolados de uma mesma raça, que desestabilizam a resistência das cultivares comerciais. Portanto, o objetivo com este trabalho foi a identificação de raças fisiológicas de isolados de C. lindemuthianum coletados no Estado de São Paulo e Santa Catarina, Brasil e, detectar a ocorrência de variabilidade patogênica entre isolados pertencentes à raça 65. A classificação de 51 isolados resultou na identificação de 10 diferentes raças fisiológica: 4, 38, 55, 65, 73, 81, 83, 85, 321 e 351, com destaque para a raças 65 e 81, que apresentaram uma frequência de 37,25 e 35,29%, respectivamente. Com relação à avaliação entre os isolados da raça 65, ficou evidente a alta variabilidade dos mesmos, sugerindo a utilização de um novo conjunto diferenciador para detectar os níveis de variação entre isolados de uma mesma raça do patógeno.Palavras-chaves: variabilidade fisiológica, níveis de variação, antracnose.

Colorectal cancer is the third most common cancer worldwide, accounting for more than 610,000 mortalities every year. Prognosis of patients is highly dependent on the disease stage at diagnosis. Therefore, it is crucial to investigate molecules involved in colorectal cancer tumorigenesis, with possible use as tumor markers. Heparan sulfate proteoglycans are complex molecules present in the cell membrane and extracellular matrix, which play vital roles in cell adhesion, migration, proliferation, and signaling pathways. In colorectal cancer, the cell surface proteoglycan syndecan-2 is upregulated and increases cell migration. Moreover, expression of syndecan-1 and syndecan-4, generally antitumor molecules, is reduced. Levels of glypicans and perlecan are also altered in colorectal cancer; however, their role in tumor progression is not fully understood. In addition, studies have reported increased heparan sulfate remodeling enzymes, as the endosulfatases. Therefore, heparan sulfate proteoglycans are candidate molecules to clarify colorectal cancer tumorigenesis, as well as important targets to therapy and diagnosis.

RESUMOO objetivo do estudo foi investigar a prevalência e os fatores associados à lesão do septo nasal em prematuros sob uso de ventilação não invasiva. Estudo transversal, cujos dados foram coletados entre março e julho de 2012 por meio de busca em prontuários, entrevistas às mães e avaliação nasal de 47 prematuros, na Unidade Neonatal de um Hospital Universitário da cidade do Recife, Pernambuco, região nordeste do Brasil. Realizou-se análise estatística descritiva e bivariada por meio do teste qui-quadrado ou Exato de Fisher, utilizando o software SPSS. A prevalência de lesão nasal foi de 68,1%, associada ao baixo peso ao nascer e à duração do tratamento. A prevalência de lesão nasal na população estudada é elevada e associa-se ao baixo peso ao nascer e ao tempo de permanên-cia em ventilação não invasiva. Infere-se a necessidade de ações preventivas, como cuidados de Enfermagem contínuos, adequação dos dispositivos e educação permanente em serviço.
