13Лапароскопия произвела революцию в лечении врожденных и приобретенных заболеваний органов брюшной полости у детей и вызвала переоценку многих клинических стратегий в детской хирургии. С появлением камер высокого разрешения и высо-кокачественных экранов эндохирургия претерпела значительные изменения в области визуализации внутренних объектов [1][2][3]. Тем не менее обычная лапароскопия все еще имеет ограничения. Двумер-ная (2D) визуализация считается ее главной слабо-стью. Одним из основных недостатков, присущих традиционной лапароскопии, является отсутствие глубины восприятия. Это упущение было устранено с появлением робот-ассистированных комплексов, которые первыми стали использовать трехмерный (3D) эффект изображения [4]. Высокая стоимость оборудования и отсутствие обратной тактильной связи являются определенными ограничивающими факторами роботизированной технологии. Совре-менная система 3D для выполнения традиционных торакоскопических и лапароскопических операций предлагает большинство из достоинств робот-асси-стированной хирургии при низкой стоимости си-стемы и возможности использования обычного ла-пароскопического оборудования [5].Однако 3D-видеосистемы до сих пор не были широко распространены в хирургии, особенно в педиатрической практике. Подобная тенденция об-условлена главным образом тем, что большинство детских хирургов удовлетворены двумерной карти-ной изображения, которая существует при выполне-нии стандартной лапароскопии. Другое объяснение этого явления заключается в отсутствии до настоя-щего времени необходимого оснащения для реали-зации 3D-лапароскопии у детей. Детские хирурги столкнулись с тем, что доступные им 3D-телескопы имеют диаметр 10 мм, который в силу существующих представлений является большим для детей. В насто-ящем исследовании мы обобщаем собственный опыт выполнения лапароскопических операций в трех- Цель исследования -доказательство возможности применения 3D-лапароскопии в педиатрической практике. Наш опыт основывается на проведении 48 эндохирургических операций в трехмерном формате, выполненных с января по декабрь 2014 г. В научной работе продемонстрирована успешность и эффективность 3D-лапароскопии у пациентов с паховой грыжей, врожденным гипертрофическим пилоростенозом, гастроэзофагеальным рефлюксом, гидронефрозом, кистой яичника и мультикистозной дисплазией почки.

Our experience based on performance of 48 endosurgical operations in three-dimensional format between January 2014 and December 2014. We demonstrated in this study a success of 3D laparoscopy at patients with inguinal hernia, congenital hypertrophic pyloric stenosis, gastroesophageal reflux, hydronephrosis, ovarian cyst and multicystic kidney.
Introduction. The authors present a clinical case of an 8-year-old patient with an extremely rare Allgrove’s syndrome and cardia achalasia. By various authors, in case of this syndrome, achalasia of the cardia is diagnosed in 75% of cases, along with other symptoms. Currently, there are a number of modalities for treating achalasia of the cardia. However, the most popular one is cardiomyotomy by Heller in combination with fundoplication by Dor which recently has been successfully performed with minimally invasive laparoscopic techniques.Clinical case. In present case, the patient had all possible minimally invasive surgical interventions. However, all attempts to apply organ-preserving techniques for treating achalasia of the cardia had failed .Results. Finally, the child had esophageal plastic surgery – coloesophagoplasty with good clinical outcomes.
