CS rates are generally very high in Brazil. They are significantly higher than the average among women attending private/insured antenatal care, among the highly educated, and in provinces with higher socioeconomic levels.
Observational studies suggest association between low concentrations of omega-3 family fatty acids and greater risk for post-partum depression (PPD). The objective was to investigate the effect of unbalanced dietary intake of omega-6/omega-3 ratio >9:1 in the prevalence for PPD. The study comprises a prospective cohort with four waves of follow-up during pregnancy and one following delivery. PPD was evaluated according to the Edinburgh Post-partum Depression Scale (PPD ≥ 11) in 106 puerperae between 2005 and 2007, in Rio de Janeiro, Brazil. Independent variables included socio-demographic, obstetric, pre-pregnancy body mass index (BMI) and dietary intake data, which were obtained by means of a food frequency questionnaire in the first trimester of pregnancy. Statistical analysis involved calculation of PPD prevalence and multivariate Poisson regression with robust variance. PPD prevalence amounted to 26.4% [n = 28; confidence interval (CI) 95%: 18.0-34.8], and higher prevalences of PPD were observed in women who consumed an omega-6/omega-3 ratio >9:1 (60.0%) and in those with pre-pregnancy BMI <18.5 kg/m(2) (66.7%). These variables held as factors associated to PPD in the multivariate model, elevating the chances of occurrence of the outcome in 2.50 (CI 95%: 1.21-5.14) and 4.01 times (CI 95%: 1.96-8.20), respectively. Analyses were adjusted for age, schooling, pre-pregnancy BMI, lipids consumption and time elapsed since delivery. It verified an association between omega-6/omega-3 ratio above 9:1, the levels recommended by the Institute of Medicine, and the prevalence of PPD. These results add to the evidence regarding the importance of omega-6 and omega-3 fatty acids in the regulation of mental health mechanisms.
Resumo Objetivou-se avaliar a ambiência escolar e o estado nutricional de pré-escolares da rede pública de Macaé. Trata-se de um estudo transversal, com dados antropométricos secundários, coletados entre 2012 e 2014. Foram analisadas 962 crianças, de 2,1 a 6,6 anos, de quatro escolas, segundo recomendação da Organização Mundial da Saúde. A ambiência foi avaliada, qualitativamente, por observação direta. Por regressão logística, foi verificado se a chance de ocorrer desvios nutricionais diferia entre as escolas. A prevalência de excesso nutricional foi alarmante, principalmente nos menores de cinco anos (13,7%). Em crianças com cinco anos ou mais, as duas escolas, com ampla rede social de apoio no entorno, tiveram menor chance de ocorrer excesso nutricional (Odds Ratio de 0,40 e 0,33; p < 0,05). A escola com mais lanchonetes e propagandas de alimentos no território teve maior proporção de obesidade. Fatores positivos de ambiência dentro da escola, como ausência de cantina comercial, não conseguiram evitar o excesso nutricional. Este estudo adverte a respeito da relevância do entorno, sobretudo do apoio social. Esse é pouco abordado na literatura em nutrição, mas pode ser diferencial no perfil nutricional, em especial quando aspectos-chave de proteção no interior da escola estão adequados.
Resumo Introdução A alimentação no âmbito do Programa Nacional de Alimentação Escolar (PNAE) permite atender às necessidades nutricionais de estudantes no ambiente escolar por meio da alimentação saudável e balanceada, contendo também alimentos da agricultura familiar (AF). O estudo objetiva descrever as preparações ofertadas na alimentação de estudantes e os alimentos oriundos da AF no PNAE do município de Codó, no Maranhão. Método Trata-se de um estudo descritivo, de base escolar, contemplando as escolas de ensino fundamental da rede pública da área urbana, vinculadas ao PNAE, entre março e junho de 2015. Resultados Todas as escolas (n=26) participaram do estudo. Das preparações oferecidas, 16% atenderam ao cardápio planejado. Constatou-se a presença de pelo menos um alimento oriundo da AF em mais da metade das escolas visitadas. Conclusão A maioria das preparações oferecidas não atendeu ao cardápio planejado. Os alimentos oriundos da AF foram: iogurte, bolo, frutas, feijão, arroz, entre outros.
