PurposeTo report characteristics of our treatment scheme of high-dose-rate (HDR) brachytherapy of breast cancer and to show the first outcomes of dosimetric planning analysis based on dose-volume histogram (DVH).Material and methodsSince August 2017, 25 patients diagnosed with T1N0M0 breast cancer have received a treatment in our center. There was lumpectomy and following breast HDR brachytherapy (10 fractions of 3.4 Gy) administered to each patient. A planning target volume (PTV) and organs at risk (OARs) were recorded with DVH analysis.ResultsThe study describes the full procedure of breast HDR brachytherapy with the lumpectomy. Twenty-five patients were treated, including 9 with cancer of the left breast and 16 of the right breast. The median age was 65 years. The first analysis of DVH data shows that the main OARs were ribs and skin. Mean value of Dmax (ribs) for all patients was 19.90 Gy (55.88% of prescribed dose) and for the skin 30.88 Gy (90.74% of prescribed dose). During the treatment, there was only one case of toxic effects, which was pigmentation on the skin due to excess of dose limit of 1.4 Gy. Therefore, the limit exceeding of 1 Gy does not give any significant toxic effects.ConclusionsThis study is the first stage of the dosimetric evaluation of a new method. The analysis allows treating complex localizations of the breast cancer, for example, in a close position to the skin or ribs. In order to minimize the toxic effects, it is necessary to consider patient selection, catheter administration, and dose optimization.
Рак предстательной железы (РПЖ) является одним из наиболее распространенных злокачественных заболеваний у мужчин во всем мире. В России доля больных РПЖ I-II стадии выросла с 37,6 до 56,0 %, что, в свою очередь, позволяет все большему числу пациентов выполнять радикальные методы лечения. Неуклонно растет число пациентов как с рецидивом РПЖ после лучевых методов лечения (дистанционной лучевой терапии (ДЛТ) или брахитерапии), так и с локальным рецидивом после радикальной простатэктомии (РПЭ). Цель исследования-улучшить результаты лечения пациентов с рецидивом РПЖ. Материалы и методы. В МРНЦ им. А. Ф. Цыба-филиале НМИЦ радиологии с 2016 г. проводится лечение больных с рецидивом РПЖ с помощью спасительной высокомощностной брахитерапии (СВБТ) с использованием 192 Ir после радикальных лучевых методов лечения (ДЛТ или брахитерапии) или с локальным рецидивом после РПЭ. Пациенты были разделены на 3 группы в зависимости от вида первоначального радикального лечения: РПЭ, ДЛТ и брахитерапии. Результаты. Наиболее благоприятные результаты СВБТ были зарегистрированы в группе пациентов с местным рецидивом после РПЭ: у 7 (87 %) из 8 больных через 1 год после проведенной СВБТ уровень простатического специфического антигена (ПСА) составил ˂0,1 нг / мл. У 1 пациента уровень ПСА через 2 года после спасительного лечения снизился до 0,71 нг / мл (исходно 2,47 нг / мл). В настоящее время отмечается дальнейшее снижение уровня ПСА. Результаты после выполнения СВБТ у пациентов с рецидивом после ДЛТ: у 2 (25 %) из 8 пациентов через 1 год уровень ПСА составил ˂0,2 нг / мл. У 4 пациентов уровень ПСА через 2 года после лечения-˃2,0 нг / мл. У данных пациентов зарегистрирован рецидив. Еще у 2 пациентов также отмечен повторный рецидив после СВБТ. После проведения СВБТ у пациентов с рецидивом после брахитерапии (низко-и высокомощностной) получены следующие результаты. У 7 (58 %) из 12 пациентов через 2 года после СВБТ уровень ПСА составил ˂0,5 нг / мл. СВБТ выполняли в режиме 2 фракций по 12,5 Гр каждая с интервалом 2 нед. У 3 пациентов уровень ПСА через 2 года после спасительного лечения превысил 0,5 нг / мл, в связи с чем у них определяется повторный рецидив. Еще у 2 пациентов также отмечен повторный рецидив после проведения СВБТ. Заключение. Приведены предварительные результаты нашего исследования. В качестве опций для лечения локального рецидива РПЖ возможно выбрать спасительную простатэктомию, спасительную лучевую терапию, гормональную терапию или СВБТ. В некоторых случаях у пациентов данной категории применяют ДЛТ, однако это может привести к развитию тяжелых лучевых повреждений. Выбор метода лечения в случае рецидива РПЖ является сложной проблемой, решение которой основывается на многих факторах. Учитываются клиническое течение РПЖ, степень распространенности заболевания, уровень маркеров, молекулярно-генетические данные.
The study purpose was to demonstrate developed treatment scheme for breast cancer high-dose-rate brachytherapy with 192Ir and evaluate received radiation exposure. The results of re-search are discussed in the paper. Thirty patients of median age of 63 years with diagnosed T1N0M0 breast cancer received treatment between August, 2017 and June, 2019 in A. Tsyb MRRC. All patients underwent lumpectomy followed by accelerated partial breast irradiation (APBI) with single radiation dose of 3.4 Gy and total dose of 34 Gy. A patient received two frac-tions of irradiation daily, the interval between them was no less than 6 hours. The treatment dura-tion was 5 days. The treatment outcomes were assessed in terms of the treatment planning in-cluding radiation exposure to the target, healthy tissues and organs-at-risk. The study highlighted that determined dosimetric parameters correspond to those given in the approved treatment pro-tocol and meet the international recommendations. As the result of this research it is shown that treatment plan for each patient should be developed individually. Since the follow-up period is small, further studies will be conducted. This will allow early detection of toxic effects of brachy-therapy and the analysis of living status.
