BackgroundQuality of life (QOL) is an important component in the evaluation of the well-being of people living with HIV and AIDS (PLWHA), especially with the appreciable rise in longevity of PLWHA. Moreover, limited studies have been conducted in Nigeria on how PLWHA perceive their life with the World Health Organisation Quality of Life Brief Scale (WHOQOL-Bref) instrument.ObjectiveThis study assessed the QOL of PLWHA attending the antiretroviral (ARV) clinics, UCH Ibadan, Nigeria.MethodA cross-sectional study was conducted from June to September 2008 that involved 150 randomly selected HIV-positive patients who were regular attendees at the antiretroviral clinic, UCH Ibadan. An interviewer administered questionnaire was used to collect information on sociodemographic data, satisfaction with perceived social support, medical records, and QOL was assessed with WHOQOL-Bref.ResultsThe mean age of the respondents was 38.1 ± 9.0 years and the male: female ratio was 1:2. The mean CD4 count was higher in female patients than in male patients, 407 cells/mm3: 329 cells/mm3 (p = 0.005). The mean QOL scores on the scale of (0–100) in three domains were similar: psychological health, 71.60 ± 18.40; physical health, 71.60 ± 13.90; and the environmental domain, 70.10 ± 12.00; with the lowest score in the social domain, 68.89 ± 16.70. Asymptomatic HIV-positive patients had significantly better mean QOL scores than symptomatic patients in the physical (74.04 ± 16.85 versus 64.47 ± 20.94, p = 0.005) and psychological domains (76.09 ± 12.93 versus 69.74 ± 15.79, p = 0.015). There was no significant difference in the mean QOL scores of men compared to those of women, in all domains assessed.ConclusionHigh QOL scores in the physical, psychological and environmental domains may be reflective of the effectiveness of some of the interventions PLWHA are exposed to at the ARV clinic, UCH Ibadan (on-going psychotherapy, free antiretroviral drugs). Relatively low social domain scores may suggest ineffective social support networks, because PLWHA are still exposed to stigmatisation and discrimination. An improvement in social support for PLWHA, therefore, will improve their quality of life further.

A significant burden of dyslipidemia exists among ART-naive HIV-infected persons. Low HDL-C was the most frequently observed abnormality. The abnormalities related more with viral load levels than with CD4 counts. Dyslipidemia screening should be done in ART-naive HIV-infected persons. Simple healthy lifestyle changes should be emphasized, with other care given to those with the disorder.

BackgroundThe family as a unit of care has great effect in tackling adolescent problems and this could be influenced by family functioning.ObjectiveThis study assesses the relationship between adolescents’ family functioning with socio-demographic characteristics and behavioural risk factors.MethodThe research was a cross-sectional, hospital-based study carried out at the General Outpatients Department, University College Hospital (GOPD, UCH), Ibadan, over a period of three months. Four hundred subjects were recruited using a modified Guideline for Adolescent Preventive Services (GAPS) questionnaire, with an incorporated family APGAR (Adaptation, Partnership, Growth, Affection, Resolve) score table. The results were analysed using the Statistical Package for Social Sciences (SPSS), version 11 and the findings on the family assessment and behavioural risk factors were relayed to the respondents.ResultsThe ages of the adolescents ranged from 10 to 19 years. Of the subjects, 8% were sexually active. Mean age for first coitus among the respondents was 15 ± 2.4 years. The rate of ingestion of alcohol and cigarette smoking was very low. The family APGAR scores obtained revealed that 84.5% subjects were rated as having a functional family (7–10 points) and 15.5% of the subjects were rated as having a dysfunctional family (0–6 points). There was a significant association between perceived family function and subjects’ occupation (p = 0.01), parent social class (p = 0.00) and subjects’ sexual activities (p = 0.00).ConclusionThe majority of the adolescents were rated as having functional families. Dysfunctional families had significantly sexually active respondents.

The onset of early otitis media (EOM), in the first few months of life has been reported to predict later chronic otitis media (CSOM), although the prevalence rates are increasing little is known about specific risk factors. In this survey we examined the hypothesis that higher risk factors is associated with the development of OM within 1 year compared to later onset and early onset otitis media (OM) has potential for negative outcome of CSOM. This is a survey of the age at onset of otorrhoea and associated risk factors in children with CSOM, in five sites spread in two sub-urban cities in two states in Nigeria. Questionnaires were administered on the informants followed by examination of the children. EOM was seen in 136/189 (70%) with CSOM, the age range was 1-150 months, mean of 59.25 (SD = 44.55). Of the 85 CSOM subjects with hearing loss, EOM accounted for 49 (57.7%) while 36 (42.4%) was later onset, On multivariate analysis (OR = 0.276, CI = 0.133-0.572, P = 0.001) revealing EOM was significant in the development of hearing loss however there was no correlation with the frequency of attack of otorrhoea (OR = 1.025, CI = 0.88-1.19, P = 0.75). Low socioeconomic status seen in 110/136 EOM (P = 0.000), allergy (P = 0.030) and number of people >10 in household (OR = 4.13, CI = 1.81-9.39, P = 0.001) constituted the significant risk for EOM compared to later onset. Bottlefeeding, adenoiditis/adenoid hypertrophy, indoor cooking and upper respiratory infection were not found to have statistical significance in early onset OM compared to later onset OM. This study found correlation between EOM and hearing loss and identified allergy, low social status and chronic exposure to overcrowding through increased number of children in the household significant risk factors for future research focus. This may help in controlling the prevalence of hearing loss accompanying CSOM.

Jugular vein phlebectasia is a rare venous anomaly commonly presenting as a unilateral neck swelling in children and adults. Its aetiology is unknown and its detection may have increased with availability of better imaging facilities. Due to its rarity, a high index of suspicion is required once other causes of neck swelling are excluded. The few reported cases are mainly in children and non-Africans adults. The authors report the case of a 54-year-old Nigerian woman presenting for the first time with a progressive, intermittent, painless right neck swelling, which was demonstrated on Doppler ultrasound and CT angiography to be an internal jugular phlebectasia. She had satisfactory surgical excision of the dilated segment for cosmetic reasons.
