Vitrectomy With Silicone Oil or Sulfur Hexafluoride Gas in Eyes With Severe Proliferative Vitreoretinopathy: Results of a Randomized Clinical Trial

“…Historically, the visual recovery for patients with recurrent retinal detachments from grade C1 PVR has been poor. Final visual acuity after relaxing reti-notomy for PVR in 1990 was >5/200 in 26% of patients as reported by Han et al 29 and >3/200 in 32% of patients as reported by Iverson et al 30 This improved in the early 2000s, although De Silva still reported 51% of eyes having a fi nal visual acuity of <20/400, 22 similar to other contemporaneous studies showing relatively low fi nal acuities. 25,31,32 Although 88% of our patients had detachments that involved the macula, 22 of 24 eyes (92%) in our study had a measurable improvement in vision after surgery with the MTX protocol, with vision recovery to 20/60 or better in 6 of 24 eyes (25%) at 6 months and 5 of 20 eyes (25%) at 12 months, and improvement to 20/200 in 18 of 24 eyes (75%) at 6 months and 16 of 20 eyes (80%) at 12 months.…”

A four-year retrospective surgical case series examining postoperative complications following removal of silicone oil (ROSO) scored using the Complications in Retinal Detachment Surgery (CORDS) Severity Classification Tool

“…These gures are higher than our study gures but are likely explained by the earlier removal of SO tamponade (between 9.1-20.2 weeks) and the more extremis patient cohort (GRT and advanced PVR) [5]. The Silicone Study of 1985-1990 [33] achieved ROSO in 99 eyes (8 weeks minimum of SO tamponade) and reported the following postoperative complication frequencies: retinal re-detachment (18%), loss of VA (20%), raised IOP (1%), hypotony (16%) and keratopathy (20%) [8]. A more recent study [17] reported on 101 eyes that underwent RD repair, irrespective of aetiology, with a mean ROSO time of 9.46 months.…”
