Uterine leiomyomas are the most common tumors of the female genital tract, affecting 50% to 70% of females by the age of 50. Despite their prevalence and enormous medical and economic impact, no effective medical treatment is currently available. This is, in part, due to the poor understanding of their underlying pathobiology. Although they are thought to start as a clonal proliferation of a single myometrial smooth muscle cell, these early cytogenetic alterations are considered insufficient for tumor development and additional complex signaling pathway alterations are crucial. These include steroids, growth factors, transforming growth factor-beta (TGF-β)/Smad; wingless-type (Wnt)/β-catenin, retinoic acid, vitamin D, and peroxisome proliferator-activated receptor γ (PPARγ). An important finding is that several of these pathways converge in a summative way. For example, mitogen-activated protein kinase (MAPK) and Akt pathways seem to act as signal integrators, incorporating input from several signaling pathways, including growth factors, estrogen and vitamin D. This underlines the multifactorial origin and complex nature of these tumors. In this review, we aim to dissect these pathways and discuss their interconnections, aberrations and role in leiomyoma pathobiology. We also aim to identify potential targets for development of novel therapeutics.

Vitamin D deficiency has an impact on the reproduction of more than 40% of reproductive age women globally. Fibroids are more common among African-American females owing to their decreased milk consumption and reduced absorption of ultraviolet rays, supporting the relation between vitamin D deficiency and fibroid development. Vitamin D has an inhibitory effect on leiomyoma cells by suppression of proliferation cell nuclear antigen (PCNA), BCL-2, BCL-w, CDK1, and catechol-Omethyltransferase (COMT) protein levels. A growing evidence support the relationship between vitamin D deficiency and endometriosis through overexpression of vitamin D recseptor (VDR) and α-hydroxylase enzyme, however, it is still unclear if the endometriosis patients could benefit from vitamin D supplementation. Effect of vitamin D supplementation on the metabolic outcomes of polycystic ovary (PCO) has been studied and reveled that it is negatively correlated with fasting glucose, fasting insulin, triglycerides, C-reactive protein, free androgen index, and Dehydroepiandrosterone (DHEAS) and positively associated with quantitative insulin sensitivity check index (QUICKI), high density lipoprotein cholesterol (HDL-C), and sexual hormone binding globulin (SHBG), whereas its impact on the ovarian function is still unclear. Vitamin D deficiency may worse the obstetrical outcomes, including preeclampsia, gestational diabetes, low birth weight, increased cesarean section rate, neonatal asthma, seizures, and preterm labor. The relationship between serum levels of 25-hydroxy-vitamin D (25(OH) D) and pregnancy rates in ART is still debatable, with the need to conduct more clinical trials toward it. The in vitro antiproliferative and prodifferentiative effect of vitamin D might find a role in control of hyperplastic overactive bladder. Several studies support that vitamin D deficiency constitutes a risk factor for development of many types of cancer such as breast, ovarian, and colorectal.

StreszczeniePowszechnie znane jest korzystne działanie suplementacji witaminą D na układ kostny. Badania naukowe ostatnich lat donoszą o pozytywnym wpływie witaminy D na wiele chorób układu sercowo-naczyniowego, układu nerwowego, układu mięśniowego, chorób metabolicznych i nowotworów (piersi, jelita grubego, prostaty). Prawidłowa podaż witaminy D mierzona jest stężeniem 25(OH)D w surowicy. U osób dorosłych wartość powinna oscylować między 30 a 80 ng/ml. Profilaktyczna dawka witaminy D dla osób dorosłych wynosi 800-1000 IU/dobę. Zaleca się stosowanie jej od października do marca oraz w miesiącach letnich, kiedy synteza endogenna i podaż pokarmów nie pokrywają dziennego zapotrzebowania na witaminę D.Słowa kluczowe: witamina D, choroby układu sercowo-naczyniowego, rak piersi, choroby układu mięśnio-wego, mózg. AbstractVitamin D supplementation plays an important role in the skeleton and calcium metabolism. The results of studies published in the 21 st century suggest that vitamin D supports the function of the heart, brain, muscle and plays an important role in cancer (e.g. breast, colorectal, prostate cancer). Serum concentration of 25-OHD is a biomarker of the vitamin D status. The optimal concentration of 25-OHD is between 30 and 80 ng/ml. The daily dose of vitamin D should be between 800 and 1000 IU. For adults, vitamin D supplementation is suggested from October to March.
