ÖZETVücut ısısının sabit tutulması ısı üretimi ve ısı kaybı arasındaki dengenin sağlanması ile olur. Yenidoğan bebek homeotermiktir, yani ısı kaybı ve ısı üretimi dengededir. Çevre ısısının aşırı düşük/yüksek oluşu ısı kaybı ve ısı üretimi arasındaki dengeyi bozar. 28 haftanın altında doğan prematüre bebeklerde yetersiz kreatinli tabaka ve yüksek transepidermal su kaybı olarak tanımlanan olgunlaşmamış epidermal bariyer vardır. Prematüre bebekler özellikle hipotermiye karşı hassastır ve bu durum kahverengi yağ dokusunun yokluğuna ya da az olmasına, ısıyı korumak için gereken hareket, titreme ve esneme kısıtlılığına bağlıdır. Prematüre bebeklerde mortalite ve morbidite oranını azaltmak için doğumhanelerde özellikle resüstasyon sırasında ısı kontrolü önemlidir. Güncel neonatal resüstation program (NRP) yönergelerinde alınan önlemlere rağmen düşük doğum ağırlıklı (DDA) prematüre bebeklerde soğuk stresi riski yüksektir. Isı transferini azaltmak için bu yöntemlerin terk edilmemesi ancak ilave yöntemlerle desteklenmesi gerekmektedir. Bu makalede DDA prematüre bebeklerde hipotermiyi önlemede plastik örtü kullanımının etkilerinden bahsedilmektedir. DDA prematüre bebeklerde polietilen örtü kullanarak hipoterminin önlendiği ve bebeğin vücut ısısının korunduğu çalışmalarla gösterilmiştir. Prematüre bebekleri doğumhanede polietilen torbaya koymanın veya polietilen örtüyle sarmanın hipotermi oranını belirgin bir şekilde azalttığı ve ortalama vücut ısısını 1˚C arttırdığı belirlenmiştir. Bu yöntemin DDA prematüre bebeklerde hipotermiyi engellemede etkili ve ucuz bir yöntem olduğu çalışmalarda bildirilmektedir.Anahtar Kelimeler: Prematüre, hipotermi, plastik örtü SUMMARYIt is possible to stabilize the body temperature by acquiring a balance between heat production and loss. A new born baby is homeothermic, which means a balance between heat loss and heat production. It causes a destabilization between production and loss if the outside temperature is too low or too high. Premature babies who were born before 28 weeks have an inadequate creatine layer and an immature epidermical barrier which is described as high transepidermal water loss. Premature babies are especially sensitive to hypothermia. This is because they have no or little amount of brown adipose tissue and they have limited movements, shivering and bending behaviours. Controlling the temperature in the delivery room during resuscitation is especially vital in order to decrease the rate of mortality and morbidity in premature babies. Despite the measures taken as indicated in recent neonatal resuscitation program (NRP) instructions, the risk of cold stress is higher in premature babies with low birth weight (LBW). In order to decrease heat transfer, it is required to carry on these measures but to support them with additional ones.
