Surgeons’ mental and physical workloads are major focuses of operating room (OR) ergonomics, and studies on this topic have generally focused on either mental workload or physical workload, ignoring the interaction between them. Previous studies have shown that physically demanding work may affect mental performance and may be accompanied by impaired mental processing and decreased performance. In this study, 14 participants were recruited to perform laparoscopic cholecystectomy (LC) procedures in a virtual simulator. Surface electromyography (sEMG) signals of the bilateral trapezius, bicipital, brachioradialis and flexor carpi ulnaris (FCU) muscles and eye-tracking signals were acquired during the experiment. The results showed that the least square means of muscle activity during the LC phases of surgery in an all-participants mixed effects model were 0.79, 0.81, and 0.98, respectively. The observed muscle activities in the different phases exhibited some similarity, while marked differences were found between the forearm bilateral muscles. Regarding mental workload, significant differences were observed in pupil dilation between the three phases of laparoscopic surgery. The mental and physical workloads of laparoscopic surgeons do not appear to be generally correlated, although a few significant negative correlations were found. This result further indicates that mental fatigue does markedly interfere with surgeons’ operating movements.

Purpose: The purpose of this integrative literature review is to examine the evidence on factors affecting patient safety during robotic-assisted and laparoscopic surgeries. Design: Systematic review of papers published between 2011 and 2016 that identified factors affecting safety during robotic-assisted and laparoscopic surgeries, in the areas of colorectal, general, urological, and gynecological surgeries. Methods: A systematic literature search of the Cumulative Index to Nursing and Allied Health Literature (CINAHL), Embase, and Medline databases were performed. Twelve studies met the inclusion criteria outlining factors affecting safety in robotic-assisted and laparoscopic surgeries. All 12 studies selected were quality appraised using the Critical Appraisal Skills Programme. Findings: Using thematic analysis, the outcomes from the 12 studies were categorized into three thematic categories. Intraoperative communication, teamwork, and disruptions are the key factors affecting patient safety during robotic-assisted and laparoscopic surgeries. Conclusions: This integrative literature review identifies a dearth of evidence examining factors affecting patient safety during robotic-assisted and laparoscopic surgeries. It draws attention to the complexities with teamwork, intraoperative communication and disruptions during robotic-assisted and laparoscopic surgeries. Although robotic-assisted surgery is generally seen as safe and effective, this review highlights the need for education and training that focuses on non-technical skills development, disruption prevention and alertness in anticipating and minimising risk. Clinical Relevance: The evidence from this review identifies the different demands and diverse challenges in maintaining safety during roboticassisted and laparoscopic surgery. Although specific technical knowledge and skills are essential, this review highlights the importance of developing new ways of thinking with regard to assessment and management of disruptions, developing different teamwork patterns and communication skills, and overcoming challenges introduced during technology advanced surgeries. Nurses in the perioperative setting have an increased responsibility to continue professional development and remain vigilant to factors affecting patient safety. Early identification and management of factors leading to disruptions is imperative in the provision of safe perioperative care.Mathew et al.

Within the framework of learning from errors this study focused on how operative risks and potential errors are addressed in guidance to surgical residents during authentic surgical operations. The overall purpose is to improve patient safety and to diminish medical complications resulting from possible operating errors. Further in the process of the optimal contexts for instruction aimed at preventing risks and errors in the practical hospital environment was evaluated. The five authentic surgical operations were analyzed, all of which were organized as training sessions for surgical residents. The data (collected via video-recoding) were analyzed by a consultant surgeon and an education expert working together. The results showed that the risks and potential errors in the surgical operations were rarely addressed in guidance during operations. The guidance provided mostly concerned technical issues, such as instrument handling, and exploration of critical anatomical structures. There was little guidance focusing on situation-based risks and potential errors, such as unexpected procedural challenges, teamwork and practical decision making. The findings showed that optimal context of learning about risks and potential errors of surgical operation is not always the authentic operation context. The study was conducted in an authentic surgical operation-cum-training context. The originality of the study derives from its focus on guidance related to risk and error prevention in surgical workplace learning. The findings can be used to create a meaningful learning environment-including powerful guidance-for practice-based surgical learning, maximally addressing patient safety, but giving possibilities also for other training options.
