Dr Mahmuda Siddiqua E-mail : mahmuda99@yahoo.com hepatitis, occurring in patients with no other known etiology. 5It is well established that HGV maintains a high and stable level of plasma viremia for many years, 6 which indicates it's efficient and active replication. Before the establishment of antibody detection methods, prevalence rates of HGV RNA in general population varied between less than 1 to 4% in different countries.7 With the development of new tests for detection of anti-HGV antibodies about 3 to 20 % of healthy people are found to have antibodies, indicating that the virus is more widely spread worldwide. In particular, rates of HGV seropositivity have ranged between 2-8% in Asia and North America, between 10-15 % in Europe and around 20% in South Africa and South America.

Effect of Primer Selection on Estimates of GB Virus C (GBV-C) Prevalence and Response to Antiretroviral Therapy for Optimal Testing for GBV-C Viremia

GB virus C (GBV-C; also called hepatitis G virus) is a common cause of infection associated with prolonged survival among HIV-infected individuals.The prevalences of GBV-C viremia vary widely in different studies, and there has been poor agreement among different laboratories performing GBV-C RNA detection in quality control studies. To determine the optimal method of measuring GBV-C RNA in clinical samples, samples obtained from 939 HIV-infected subjects were studied using reverse transcription (RT)-PCR methods amplifying four separate regions of the GBV-C genome. Primers amplifying the E2 coding region were 100% specific; however, their sensitivity was only 76.6%. In contrast, primers amplifying three additional conserved regions of the GBV-C genome (the 5 nontranslated region and the nonstructural protein-coding regions 3 and 5A) were more sensitive but produced higher rates of false-positive results. Using low-specificity primer sets influenced the significance of association between GBV-C viremia and response to antiretroviral therapy. Using a quantitative GBV-C RNA method, the GBV-C RNA concentration did not correlate with baseline or set point HIV RNA levels; however, a correlation between negative, low, and high GBV-C RNA levels and increasing reduction in HIV RNA following antiretroviral therapy was observed. Subjects with both GBV-C E2 antibody and viremia had significantly lower GBV-C RNA levels than did viremic subjects without E2 antibody. These studies demonstrate that accurate detection of GBV-C RNA by nested RT-PCR requires the use of primers representing multiple genome regions. Analyses based on testing with single primers do not lead to reliable conclusions about the association between GBV-C infection and clinical outcomes.

ResumoO vírus da hepatite G (HGV ou GBV-C) é um membro da família Flaviviridae. Baseado no perfil clínico e epidemiológico, este vírus pode ser adquirido principalmente por transmissão parenteral, por meio de sangue contaminado. Nós investigamos a presença do RNA do GBV-C/HGV em amostras de sangue periférico de doadores normais na ausência de marcadores como antígenos de superfície do HBV (HBsAg), anticorpos anti-HBc (anti-HBc), anticorpos anti-HCV e anticorpos anti-HIV-1/HIV-2. O RNA GBV-C foi detectado por reação de transcriptase reversa e reação em cadeia catalisada pela polimerase (RT-PCR). Foram analisadas 50 amostras de plasma e identificados 6 (12%) amostras positivas para o RNA do GBV-C. A presença de RNA GBV-C na ausência de hepatite B e C em doadores saudáveis pode indicar que este vírus é capaz de transmissão independente e não contribui para doença hepática. Palavras-chaves: GBV-C. Doadores de sangue. PCR. AbstractThe GB virus C (GBV-C)/hepatitis G virus (HGV) is a member of the Flaviviridae family. Based on the clinical and epidemiological profiles, this virus can be acquired mainly by parenteral transmission through contaminated blood. It was therefore investigated the presence of GBV-C/HGV RNA in the peripheral blood from healthy blood donors in the abscence of virus markers including HBV surface antigen (HBsAg), HBV core antibody (anti-HBc) and HCV antibody. HIV-1, HIV-2, were also investigated. GBV-C RNA was detected by reverse transcriptase and polymerase chain reactions (RT-PCR). It was detected GBV-C RNA in 6/50 (12 %) blood donors. The presence of GBV-C RNA in the abscence of hepatitis B and C infection in young patients and healthy donors could indicate that this virus is capable of independent transmission and does not contribute to liver disease.
