Cognitive impairment has repeatedly been described in bipolar disorders (BD). Serotonin (5-hydroxytryptophan; 5-HT) is possibly involved in these cognitive processes, more particularly in executive functions, learning, memory, and attention. The aim of this study was to investigate serotonergic vulnerability and its relation to cognitive functioning in healthy first-degree relatives of BD patients. We investigated the effects of an intravenous (i.v.) tryptophan (Trp) challenge and placebo on cognitive performance in 30 healthy firstdegree relatives of bipolar patients (FH) and 15 matched controls in a double-blind crossover design. A distinction was made between relatives of type I BD patients (FH I) and type II BD patients (FH II). Performances on planning, memory, attention, and psychomotor tasks were assessed 3 h after Trp infusion. After Trp, planning and attention were impaired in FH subjects but not in controls. Independent of Trp, FH subjects showed cognitive deficits on memory, focused and divided attention, and psychomotor performance. FH I subjects showed more pronounced cognitive impairments then FH II and controls. In all groups, Trp impaired memory and psychomotor performance significantly. In conclusions, cognitive deficits in FH following Trp may reflect a central 5-HT vulnerability in frontal brain areas. Independent of Trp, cognitive deficits in FH provide evidence for a trait marker for BD.

INTRODUÇÃOO aporte de técnicas mais modernas de exames para investigação do sistema nervoso central (SNC) vem influenciando de forma contundente a psiquiatria de uma maneira geral. Cada vez mais os estudos científicos desenvolvidos nesta área vêm considerando, além dos aspectos mentais propriamente ditos (como na psicologia cognitiva), seus substratos neuroanatômicos e neurofuncionais (neuropsicologia, neurobiologia) 1-4 . Desta forma, a psiquiatria pode ser inserida dentro do vasto campo das neurociências cognitivas, beneficiando-se de modelos que explicam as disfunções cognitivas com base no conhecimento da relação normal cérebro/ mente 2,5 .A neuropsicologia, que pode ser definida como uma ciência aplicada que visa estudar a repercussão de disfunções cerebrais sobre o comportamento e a cognição 6 , vem atualmente ganhando importante lugar no estudo das desordens psiquiátricas. A partir de testes, ela n ã o só fornece informações quanto ao potencial cognitivo global de um paciente, mas principalmente procura qualificar a natureza funcional dos déficits observados através da aná lise comparativa e qualitativa dos resultados obtidos, permitindo uma correlação anatomofuncional refinada. Assim, a neuropsicologia enriquece o diagnóstico clínico e ainda permite correlações com as informações advindas de outros exames complementares, que aferem atividade eletrogê nica, metabó lica e os de neuroimagem, fazendo uma ponte entre estes e o quadro clínico do paciente 7 .A depressão é um transtorno mental do humor; este grupo classicamente apresentaria remissão dos sintomas nos períodos interfásicos em contraposição ao caráter crônico da esquizofrenia, conforme a classificação cunhada por Kraepelin.
