Introduction. Placentation and autochthonous vascularization during the early period of intrauterine development in primary placental insufficiency are important stages in the establishment of embryo-haemochorial circulation. Vascular and angiogenesis dysfunctions in the early period of gestation are essential in the development of changes in placental vascular status with severe repercussions on pregnancy outcome in the first trimester of gestation. The purpose of the work. Evaluation of vascular density by the application of the anti-CD31 antibody in the chorio-villar germinal site in short-term dysregulated pregnancies. Material and methods. 184 cases were divided into 2 groups: stagnant pregnancies (SP (L1 = 144 cases) and early spontaneous abortion (ESA) (L2 = 40 cases). Control material: pregnancies resolved at social indications/desire (A) (n = 18) distributed according to gestational term (3-5 weeks), (6-9 weeks), and (10-12weeks). The estimation of the histomorphological features was performed by applying the classical conventional method with hematoxylin-eosin (H.E.), and the immunohistochemical evaluation of the vascular density in the chorio-villar profile with the help of the endothelial marker anti-CD31. The statistical relationships between fetal conceptus assessment forms, gestational term, and age were analyzed. Results. Distribution of pregnancies by profile: (SP) – 78.2% vs (ESA) – 21.8%, with vascular disturbances attested in 100% of cases by increasing the density of avascular chorionic villi in the case of stagnant pregnancies in 75% of cases. The application of immunoexpression with anti-CD31 at the level of the stroma of the chorionic villi determined PVI with a maximum mean in the control group (91.51±0.71) vs the ESA and SP groups (82.29±12.96 and 57.47±6.53, respectively). The index of vascularization with lumen (IV/L+) and vascularization index without lumen (IV/L-) were statistically significantly different (p = 0.011) for (SP). Applying the student t-test revealed statistically significant differences for PVI in most groups, except PSD vs SP at the age of 10-12 gestation of weeks (w.g.) At the same time, there are statistically significant differences between the means of vessels with lumen within PSD and SP (t31 = 5.500, p = 0.000005), and the means of vessels without lumen registered statistically significant differences in the PSD and SP groups (t12 = 3.862, p = 0.002). Conclusions. Pregnancy evolution depends on the degree of vascularization, as well as on the quality of the vascular network.

Immunohistochemical evaluation of apoptosis inducing factor in missed abortion after IVF

BACKGROUND: Apoptosis is involved both in the cyclic transformation and the functional activity of the endometrium during the menstrual cycle. Upon the onset of pregnancy, apoptosis is an important component of the placentation process. Moreover, changes in the apoptosis-inducing factor expression in the gravidar endometrium may lead to peculiarity both of trophoblast differentiation and the gestation course, regardless of the conception method. AIM: to study the expression of apoptosis-inducing factor in abortive material in non-developing pregnancy after IVF. MATERIALS AND METHODS: Histological and immunohistochemical analysis of the apoptosis-inducing factor expression were performed in 77 gravidar endometrium samples in patients with IVF pregnancy loss at 58 weeks and in 15 aborted materials that were received after abortion at similar week progression of pregnancy. RESULTS: Significant decrease in the expression apoptosis-inducing factor in gravidar endometrium of the control group was revealed in comparison with the same parameter of the main group regardless of gravidar endometrium transformation variation. The highest expression apoptosis-inducing factor was detected in the main group gravidar endometrium that consists secretive and proliferative glands and incomplete gravidar transformation of the stroma. CONCLUSIONS: High apoptosis-inducing factor expression both in the complete and incomplete gravidar transformation of the endometrium in aborted material after IVF pregnancy loss indicates the pathological apoptosis activation. Thus, the pathological apoptosis initiation occurs even before pregnancy onset in patients with infertility, endometrial dysfunction and chronic inflammatory process in the endometrium.

Morphological Features of the Endometrium Transformation in Recurrent Miscarriage

The problems of pathological diagnosis of recurrent miscarriage are relevant in modern reproductive medicine. One of the important molecular markers of endometrium is the progesterone-induced blocking factor, which is induced by progesterone under the influence of activated lymphocytes and has an immunomodulatory effect on the implantation characteristics of the endometrium. In the process of trophoblast invasion, a special role is also played stromal cell factor molecules synthesized by endometrial and trophoblast cells, as a mechanism that potentiates the susceptibility of the endometrium to the onset and development of pregnancy, trophoblast invasion and embryogenesis in general. The purpose of the study was the histological and immunohistochemical features of the transformation of the endometrium in recurrent miscarriage. It were studied 100 samples of endometrium in case of non-developing pregnancy at a development period of 5-8 weeks, 85 samples in habitual miscarriage and 15 samples in pregnancy interrupted surgically at the request of the woman. The presence of 2 non-developing pregnancies was verified in 57 cases (67,1%), the presence of 3 non-developing pregnancies in 28 cases (32,9%). A histological examination of the abortion material stained with hematoxylin and eosin was performed to verify the morphological transformation of the endometrium. Immunohistochemical study included detection of estrogen and progesterone receptors, stromal cell factor (SDF1), progesterone-induced blocking factor (PIBF). The results of histological and immunohistochemical studies showed that with a complete morphological transformation of the endometrium in the group of patients with habitual miscarriage, there is a violation of the receptor profile. The decrease in PIBF expression in the glands and the stromal component and the expression of SDF1 in the glands of the compact layer of the endometrium is a reflection of the immunological imbalance in the endometrium in recurrent miscarriage. The development of a unified morphological algorithm, taking into account the basic indicators of the processes of transformation of the endometrium with an assessment of its receptor profile and diagnostically significant immunological factors, makes it possible to verify the pathology of the endometrium at the molecular level and justify the need for using pathogenetically substantiated therapy at recurrent miscarriage.
