A 52-year-old male patient with a history of total arch replacement using the elephant trunk technique for acute aortic dissection 4 years before visited our hospital with the chief complaint of persistent fever. Chest computed tomography (CT) suggested prosthetic vascular graft infection, which was treated surgically after chemotherapy. The first surgery consisted of debridement of an abscess around the vascular graft and in the aorta around the elephant trunk, and thoracic descending aorta replacement and vacuum-assisted closure (VAC) in view of the risk of bleeding from the peripheral region of the elephant trunk. One week later, omental filling was performed as the second step. This is a very rare case of aortic abscess around the elephant trunk that could successfully be managed by graft-conserving treatment.

BACKGROUNDThe application of controlled levels of negative pressure has been shown to accelerate debridement and promote healing in many different types of wounds. The optimum level of negative pressure appears to be around 125 mmHg below ambient and there is evidence that this is most effective if applied in a cyclical fashion of five minutes on and two minutes off. It is believed that the negative pressure assists with removal of interstitial fluid, decreasing localised oedema and increasing blood flow. This in turn de creases tissue bacterial levels. Additionally, mechanical deformation of cells is thought to result in protein and matrix molecule synthesis, which increases the rate of cell proliferation. Despite the significant costs involved, the technique is said to compare favourably in financial terms with conventional treatments in the management of difficult to heal wounds.

ResumenLa terapia con presión negativa tópica (PNT) es una manera de actuar positivamente en el ambiente de la herida aplicando presión subatmosférica, acelerando el proceso de curación de la misma. Se realiza una presentación actualizada del tratamiento con PNT, su desarrollo, fundamentos teórico-prácti-cos y aplicaciones clínicas. A pesar que aun faltan estudios rigurosos que permitan evaluar la superioridad de la PNT versus otros métodos terapéuticos, la experiencia internacional ganada con su aplicación es vasta y positiva. Se presenta la experiencia con el uso de la terapia con PNT en Uruguay, ilustrándola con un caso clínico.Palabras clave: heridas, presión negativa tópica. summaRyTopical negative pressure (TNP) is considered a means to positively impact on wound's environment applying subatmospheric pressure, accelerating its healing process. InTRODuCCIÓnA pesar del conocimiento de la biología de las heridas y los nuevos tipos de curaciones disponibles en la actualidad, un gran número de heridas aun falla en curar evolucionando a la cronicidad. La terapia con presión negativa tópica (PNT) desarrollada por los Dres. L. Argenta y M. Morykwas en 1993 1 es una manera de actuar positivamente en el ambiente de la herida aplicando presión subatmosférica. La terapia reconoce diferentes nomenclaturas incluyendo: curación o terapia subatmosférica (subatmospheric dressing, subatmospheric therapy), técnica de sellado al vacío (vacuum sealing technique), cierre de heridas mediado por vacío (vacuum assisted closure), mechado al vacío (vacuum packing), entre otros. Básicamente su mecanismo de acción es a tres niveles del proceso de curación de heridas: -remoción del excedente de líquido intersticial, -trasmisión de fuerzas mecánicas que estimulan la proliferación tisular y -mantenimiento de un ambiente adecuado en la herida en cuanto a humedad y temperatura. Las primeras referencias con respecto a los métodos de curación de heridas se remontan a los "sanadores" egipcios (asu) que al aplicar una curación compuesta por grasa animal, miel y lino, probablemente no sabían que estaban utilizando una curación no adherente, osmótica y antibacteriana y con capacidad de absorción de exudados, respectivamente.2 Antes de 1960 el manejo de las heridas estaba dirigido a prevenir la infección por lo cual luego de lavarlas con antisépticos, las mismas se cubrían con una curación seca que evitaría el desarrollo bacteriano.
