Introduction: There are number of scoring systems to assess the morbidity and mortality of sick children in intensive care unit. Out of these scoring systems our study was designed to look for the utility of Sequential Organ Failure Assessment (SOFA) score which is less time consuming and simple to apply as a predictor of mortality in sick children admitted in Paediatric Intensive Care Unit (PICU). Methods: This was a prospective observational study done in PICU of Universal College of Medical Sciences, Bhairahawa, Nepal. Recruited patients were all critically sick children above one year who stayed in hospital above 72 hours and underwent all necessary evaluation, and were followed up until they were discharged or deceased. Initial SOFA score was calculated within 24 hours of admission (SOFA T0) and again calculated after 72 hours (SOFA T72). Delta SOFA score was calculated as the change in SOFA scores over 72 hours (SOFA T0 - SOFA T72). The primary outcome was in-hospital mortality. Results: When compared to outcome, the non survivors had high mean initial SOFA (T0) 11.51 ± ences ec ing 3.001 (P < 0.001), mean SOFA after 72 hours (T72) was 15.51 ± 4.026 (P < 0.001) and mean delta SOFA (T0-T72) was 4.58 ± 2.59 (P = 0.166) as compared to survivors. Delta SOFA was not significantly associated with outcome (P = 0.166). The initial SOFA score T0 > / = 11 predicted a mortality of 70.90% and SOFA T72 score of >/=15 predicted a mortality of 81.60% but delta sofa >/= 4 predicts a mortality of only 43.60%. Area under receiver operating characteristic (ROC) curve for SOFA TO was 0.769, for SOFA T72 was 0.890 and for delta SOFA was 0.604 and thus, showing excellent discriminative power for SOFA 72 for predicting mortality. Conclusions: The SOFA score demonstrated fair to good accuracy for predicting mortality when applied to sick children > 1 year admitted in PICU. Our study showed both initial SOFA T0 and SOFA at 72 hours predict mortality with good accuracy but SOFA at 72 hours is a better predictor of mortality as compared to initial and delta SOFA scores.

Pendahuluan: Society of Critical Care Medicine (SCCM) dan European Society of Intensive Care Medicine (ESICM) mengeluarkan definisi sepsis terbaru (Sepsis-3) yaitu disfungsi organ yang mengancam jiwa akibat disregulasi respon imun pejamu terhadap infeksi. Skor Sequential Organ Failure Assessment (SOFA) telah dipilih dan divalidasi sebagai sistem penilaian untuk mengukur disfungsi organ dikarenakan skor SOFA pada pasien dewasa dengan kecurigaan infeksi, sebanding atau bahkan lebih unggul daripada sistem penilaian lainnya dalam membedakan mortalitas rumah sakit. Untuk mengadaptasi definisi Sepsis-3, skor SOFA diadaptasi dan divalidasi untuk pasien anak-anak yang mengidap sakit kritis (pSOFA) dengan menggunakan kriteria yang telah disesuaikan berdasarkan usia. Hasil penelitian menunjukan perkiraan mortalitas pada pSOFA saat waktu kedatangan, hari 2, 4, 7 dan 14 setelah masuk Pediatric Intensive Care Unit (PICU) lebih baik dibandingkan skor disfungsi organ lainnya. Evaluasi serial dari skor pSOFA pada hari pertama setelah masuk PICU juga sangat baik dalam memprediksi prognosis dari pasien pediatri onkologi yang memakai ventilator selama 3 hari, anak-anak dengan sepsis di PICU dan berguna untuk memprediksi mortalitas 30 hari pada populasi PICU, namun kurang berhasil dalam memprediksi lamanya pasien untuk dirawat di PICU.

Highlight: Sepsis and septic shock cause morbidity and mortality in pediatric patients. The accuracy of pediatric sequential organ failure assessment and anion gap (pSOFA+AG) was compared with AG and pediatric logistic organ dysfunction-2 (AG+PELOD-2). The mortality assessment of pediatric septic patients showed that pSOFA was more sensitive than PELOD-2, while pSOFA+AG was not more sensitive than PELOD-2. Abstract: Sepsis and septic shock are some of the causes of morbidity and mortality (50-60%) in pediatric patients treated in intensive care rooms. This study aimed to compare the accuracy of pediatric Sequential Organ Failure Assessment (pSOFA) score combined with anion gap (AG) score to Pediatric Logistic Organ Dysfunction-2 (PELOD-2) score in the assessment of mortality in pediatric septic patients at the Resuscitation Room of Dr. Soetomo Geeneral Academic Hospital, Surabaya, Indonesia. This was a retrospective observational cohort study using pediatric sepsis diagnosis guidelines based on the 2016 Pediatric Sepsis Consensus and medical records between January-December 2018. All data of patients aged 1 month to 16 years with suspected infection at the Resuscitation Room were collected based on predisposing infections, signs of infection, and warning signs. Organ dysfunction was assessed by calculating the pSOFA+AG scores, PELOD-2 scores, and corrected anion gap (cAG) in the first 24 hours. Sepsis mortality was assessed by comparing the results of the pSOFA, pSOFA+AG, and PELOD-2. The results showed 94.9% sensitivity and 70.0% specificity (p<0.0001) in the pSOFA, 89.9% sensitivity and 71.3% specificity (p<0.0001) in the PELOD-2, 79.7% sensitivity and 65% specificity (p<0.0001) in the AG, 79.7% sensitivity and 73.8% specificity (p<0.0001) in the cAG, and 79.3% sensitivity (p<0.0001) in the pSOFA+AG. In conclusion, pSOFA was more sensitive than PELOD-2, while the use of pSOFA+AG was not more sensitive than PELOD-2 in assessing the mortality of pediatric septic patients.
