Utility of a Rapid Fecal Latex Agglutination Test Detecting the Neutrophil Protein, Lactoferrin, for Diagnosing Inflammatory Causes of Chronic Diarrhea

Abstract: The major cause of fecal neutrophils in patients with chronic diarrhea is chronic inflammatory bowel disease of the colon. The latex agglutination test for fecal lactoferrin offers a highly sensitive, specific, and simple means for detection of fecal neutrophils in these patients.

“…Several studies on the differential diagnosis of intestinal inflammatory conditions vs. noninflammatory syndromes evidenced that the fecal lactoferrin is a reliable marker of bowel inflammation with sensitivity of 90 percent and specificity of 98 percent. 9,10,12,13,22 On the other hand, calprotectin is easily measured in feces by commercially available ELISA, and it has extensively been used in CD diagnosis and monitoring. 8,15,16,23,24 None of these studies was performed in a postoperative context.…”

“…Calprotectin is a granulocyte-derived, calciumbinding protein that has been evaluated as a diagnostic assay to identify inflammatory diarrhea [121][122][123] and to quantify "inflammatory activity" [124][125][126]. Similarly, lactoferrin, a neutrophil granulocytic protein quantified by a latex agglutination assay [127] or by a polyclonal antibody-based enzyme linked immunoassay has been evaluated to discriminate between active IBD and irritable bowel syndrome [128].…”

“…1,2,17 Released by activated PMN, its presence is proportional to the neutrophil flux to the gastrointestinal tract. Playing an important role in the innate immunity as a bactericidal agent, 4,5 lactoferrin is a 76-kDa iron-binding glycoprotein that is the main component of the secondary granules of the neutrophils that degranulate during inflammatory processes.…”

“…1,2 According to a recent study, fecal lactoferrin is a more sensitive surrogate marker of endoscopic Crohn's disease (CD) activity than is Crohn's Disease activity index (CDAI) or C-reactive protein (CRP), and it is a useful tool in clinical practice to estimate disease activity and to monitor treatment response. 3 The main component of the secondary granules of activated neutrophils that degranulate during inflammatory processes, lactoferrin has an important role in the innate immune system.…”
