User characteristics, experiences and continuation rates of copper intrauterine device use in a cohort of Australian women

Abstract: Cu-IUDs were chosen for their efficacy and lack of hormones by a range of Australian women, including young and nulliparous women. While bleeding-related side effects were relatively common, overall continuation rates were high. Serious complications and failures were rare.

“…Interestingly, however, only 8 participants discontinued the IUD for this reason. 12 Overall IUD continuation rates at 1-year are high despite side effects, 70.6% in present study, 83% and 79.1% in the study with both copper and hormonal IUD cohorts and the copper only cohort study respectively.…”

“…Other studies have explored the reasons for discontinuation of both copper and hormonal IUDs, siting pelvic pain as a primary reason for discontinuation. 11,12 In a prospective study on IUD continuation among nulliparous women, the majority of participants, like participants in this study, chose the hormonal IUD. Baseline dysmenorrhea was similar in both hormonal and copper IUD groups.…”

“…This shape produces better clinical effects, and the IUD can be left in place for $20 years. IUDs, both copper and noncopper, have been used for decades as a means of contraception and have proved to be a safe and reliable method in selected patients (23,24). The mechanisms of action of IUDs are complex and are thought to be related to the induction of a local inflammatory response in the uterus that prevents implantation (25).…”

“…This was somewhat surprising because, as previously mentioned, heavy bleeding is one of the most common side effects cited for early copper IUD discontinuation. 8,9 Of note, our hypothesis was that women with heavy baseline bleeding would be at increased risk of copper IUD discontinuation. This was based on the premise that any further bleeding caused by the device would prompt its removal.…”
