Background/Aims: The diagnosis of type 2 diabetic nephropathy (T2DN) patients is important to prevent the long-term damaging effects of kidney loss in patients with diabetes and is decisive for patient outcomes. The aim of this study was to explore urine retinol binding protein (RBP) and neutrophil gelatinase-associated lipocalin (NGAL) in T2DN patients with and without albuminuria. Methods: A total of 293 T2DN patients were divided into three groups according to their urine albumin/urine creatinine ratio (UACR): normoalbuminuria group (UACR<30 mg/g, n=100), microalbuminuria group (UACR 30–300 mg/g, n=100) and macroalbuminuria group (UACR>300 mg/g, n=93); 50 non-diabetic subjects were recruited as the control group. The levels of urine RBP, NGAL, TNF-α and IL-18 in T2DN patients and non-diabetic subjects were measured using ELISA assays. Results: We first analyzed the clinical characteristics of the control and T2DN groups and found that urine NGAL, RBP, TNF-α and IL-18 levels were significantly increased and significantly correlated with the degree of albuminuria. In addition, univariate linear regression analysis showed that urine RBP was associated with UACR, BMI, Scr, BUN, TG, disease duration, SBP, NGAL, TNF-α and IL-18 levels, and urine NGAL was positively correlated with UACR, Scr, BUN, RBP, TNF-α and IL-18 levels. Conclusion: The results indicate that urine levels of NGAL and RBP may be independently associated with albuminuria in T2DN and may serve as novel biomarkers for the identification of T2DN.

Background and Aims: Urinary neutrophil gelatinase associated lipocalin (uNGAL) and urinary kidney injury molecule-1 (uKIM-1) are markers of acute kidney injury. The albuminuria is a well-known abnormality after physical exercise. The aim of this study was to investigate changes in uNGAL and uKIM-1 after intensive exercise causing albuminuria. Methods: The study population consisted of 19 participants (10 males and 9 females). The mean age of participants was 35.74 years. All were fit amateur runners; the mean body mass index was 21.99 in females and 24.71 in males. The subjects underwent a graded treadmill exercise test (GXT) according to the Bruce protocol. Maximal oxygen consumption (VO₂max) was measured. Immediately before and after the test urine was collected. Urinary creatinine, albumin, NGAL, and KIM-1 were measured. Albumin to creatinine (ACR), KIM-1 to creatinine (KCR), and NGAL to creatinine (NCR) ratios were calculated. Results: The mean VO₂max was 53.68 in females and 59.54 mL/min/kg in males. Albuminuria and ACR were significantly higher after exercise. An increase in the ACR from 8.82 to 114.35 mg/g (p < 0.01) was observed. uKIM-1 increased significantly after exercise from 849.02 to 1,243.26 pg/mL (p < 0.05). KCR increased from 1,239.1 to 1,725.9 ng/g but without statistical significance (p = 0.07). There were no statistical changes in pre- and post-run uNGAL levels. There was no correlation between post-GXT albuminuria and uKIM-1. Conclusions: uKIM-1 is a very sensitive marker of kidney dysfunction. In our study, uKIM-1 increased significantly after a very short period of exercise. It is not clear if the increase in KIM-1 is caused by post-exercise albuminuria.

В исследование вошли 87 пациентов с СД1 и 31 здоровый доброволец (контрольная группа). Больные с СД1 были разделены на подгруппы в зависимости от уровня альбуминурии -нормоальбуминурия (НАУ, n=30), микроальбуминурия (МАУ, n=29), протеинурия (ПУ, n=28), стажа СД1 -до 5 лет (n=17), 5-10 лет (n=14), 10-20 лет (n=34) и более 20 лет (n=22). Пациентам с СД1 проводилось ультразвуковое исследование (УЗИ) почечных артерий (ПА) и динамическая нефросцинтиграфия (ДНСГ). Всем участникам исследования определялось содержание сердечной формы белка, связывающего жирные кислоты (HFABP), в моче. Мочевая экскреция HFABP у больных СД1 с НАУ была выше, чем у лиц контрольной группы (p<0,001). Проведена оценка показателей почечной гемодинамики в соответствии со стажем СД1, по результатам которой создана схема этапов УЗИ ПА у лиц с СД1. Разработан оптимизированный алгоритм диагностики ДН у больных СД1, включающий определение мочевой экскреции HFABP, проведение УЗИ ПА, а также ДНСГ. The purpose of this study was to optimize the diagnostic algorithm for diabetic nephropathy (DN) in patients with diabetes mellitus type 1 (T1DM) using laboratory and radiological markers of early kidney injury. The study included 87 patients with T1DM and 31 healthy volunteers (control group). Patients with T1DM were divided into subgroups depending on the level of albuminuria -normoalbuminuria (n=30), microalbuminuria (n=29), proteinuria (n=28), and the duration of T1DM -up to 5 years (n=17), 5-10 years (n=14), 10-20 years (n=34) and more than 20 years (n=22). Patients with T1DM underwent ultrasonography of the renal arteries (RA) and dynamic renal scintigraphy. The content of heart fatty acid binding protein (HFABP) in urine was determined for all examined persons. Urinary excretion of HFABP of individuals in normoalbuminuria subgroup was higher as compared to the control group (p<0,001). The indicators of renal hemodynamics in relation to T1DM duration were evaluated, which resulted in the creation of a scheme of RA ultrasound examination for T1DM patients with the dependence on the duration of the disease.
