Urinary Markers of Tubular Injury in Early Diabetic Nephropathy

Abstract: Diabetic nephropathy (DN) is a common and serious complication of diabetes associated with adverse outcomes of renal failure, cardiovascular disease, and premature mortality. Early and accurate identification of DN is therefore of critical importance to improve patient outcomes. Albuminuria, a marker of glomerular involvement in early renal damage, cannot always detect early DN. Thus, more sensitive and specific markers in addition to albuminuria are needed to predict the early onset and progression of DN. Tub… Show more

“…Retinolbindingprotein (RBP), a low molecular weight protein (21 kDa) which belongs to the lipocalin super family, is freely filtered at the glomerulus and then almost completely reabsorbed in the proximal tubule [6, 7]. Urinary RBP has been identified as abiomarker of proximal tubular dysfunction and is important for the diagnosis of Dent’s disease, cystinosis and cast nephropathy.…”

“…These 2 markers can be measured in urine. The increase in urinary levels of NGAL as well as KIM-1 was proved to be diagnostic in AKI [13]. There are some differences between NGAL and KIM-1.…”

“…It was shown in previous studies that their levels in urine and blood are increased in AKI even before the increase in creatinine and the appearance of albuminuria [12,13]. The increase in new AKI biomarkers was documented after cardiosurgery, contrast material administration, and kidney transplantation [14][15][16].…”

“…Существует ряд молекул, которые отражают дисфункцию канальцев почек. По данным литературы к ним относятся N-ацетил-β-D-глюкозаминидаза, нейтрофильный желатиназо-ассоциированный липокаин, ретинол-связывающий белок, печеночная и сердечная формы белка, связывающего жирные кислоты [9]. Роль последнего у больных с ДН изучена недостаточно [7,14].…”
