Update on the efficacy of statin treatment in acute coronary syndromes

Abstract: Background The natural history of atherosclerosis might involve coronary plaque rupture/erosion, thrombus formation and vessel lumen occlusion, clinically recognized as acute coronary syndrome (ACS). International guidelines strongly recommend early statin administration in patients admitted for ACS. In addition to lowering circulating levels of low-density lipoprotein cholesterol (LDL-c), statin treatment was shown to promote plaque stabilization or regression in several ways, including reduction in necrotic … Show more

“…This is illustrated by hypertension and measures of blood pressure, as well as by similarly controversial interpretation of deviations in circulating levels of cholesterol, glucose and prostate-specific antigen (Robinson et al, 2014;Rosa et al, 2014). Moreover, for psychiatric disorders, the definition of "healthy" is especially complex and ambivalent since notions of normative behaviour and well-being may vary from person to person and population to population as a function of context, upbringing, religion, culture and age etc (Cloninger 2006;Harris and Deary 2011;Khoury et al, 2014).…”

“…In considering the question of what criteria of abnormal is appropriate prior to instigating preventive therapy, it is again interesting to consider the parallel with statins used for precluding onset of cardiovascular disorders based on circulating levels of cholesterol (Rosa et al, 2014). What we currently lack in psychiatry are: reliable biomarkers of risk even where appearances are "normal"; very safe drugs for treatment of vulnerable people; and large-scale studies demonstrating their benefit in an at-risk population.…”

“…The use of LLT appeared to affect the type of MI that the patients suffered, with significantly fewer of the treated group experiencing a STEMI (52.4% vs. 75.0%). In addition to lowering LDL-C levels, statins have also been shown to stabilize atherosclerotic plaque and to improve endothelial cell function 11, 12. Therefore, the prior statin treatment may have reduced the occurrence of total vessel occlusion through diminishing the extent of plaque rupture and thrombus formation, thereby making a STEMI less likely.…”

“…However, there is an increased risk of statin-associated adverse effects with such high-dose statin treatment [6]. There is a similar thought process and risks associated with using high-dose statins to prevent recurrence of thrombotic stroke [7].…”
