Aim: To describe early cerebral oxygenation (cSO 2 ) and fractional tissue oxygen extraction (FTOE) values and their evolution over the first days of life in infants with all grades of hypoxic-ischaemic encephalopathy (HIE) and to determine whether cSO 2 and FTOE measured early (6 and 12 h) can predict short-term outcome.Methods: Prospective, observational study of cerebral near-infrared spectroscopy (NIRS) in infants >36 weeks' gestation with HIE. Ten one-hour epochs of cSO 2 and FTOE were extracted for each infant over the first 84 h. Infants with moderate and severe HIE received therapeutic hypothermia (TH). Abnormal outcome was defined as abnormal magnetic resonance imaging (MRI) and/or death.Results: Fifty-eight infants were included (28 mild, 24 moderate, 6 severe). Median gestational age was 39.9 weeks (IQR 38.1-40.7) and birthweight was 3.35 kgs . cSO 2 increased and FTOE decreased over the first 24 h in all grades of HIE. Compared to the moderate group, infants with mild HIE had significantly higher cSO 2 at 6 h (p = 0.003), 9 h (p = 0.009) and 12 h (p = 0.032) and lower FTOE at 6 h (p = 0.016) and 9 h (0.029). cSO 2 and FTOE at 6 and 12 h did not predict abnormal outcome. Conclusion:Infants with mild HIE have higher cSO 2 and lower FTOE than those with moderate or severe HIE in the first 12 h of life. cSO 2 increased in all grades of HIE over the first 24 h regardless of TH status.

Introduction: Ischemic stroke, an abrupt blockage of artery, accounting for the most cases of stroke, causes high neurological mortality across the world. Recent evidence has uncovered that circular RNAs (circRNAs) highly engage in ischemic stroke-related neuronal injury. This study concentrated on a novel circRNA hsa_circ_0000304 (termed as circCELF1), trying to unveil its role and underlying mechanism in ischemic stroke. Methods: RT-qPCR and Western blot assays were conducted to detect the expression levels of RNA and protein, respectively. Functional analysis was performed to evaluate the influences of circCELF1 expression on astrocyte apoptosis and autophagy. Multiple mechanism assays were performed to probe the molecular mechanism underlying circCELF1 regulation. The oxygen-glucose deprivation/reoxygenation (OGD/R)-induced astrocytes model and transient middle cerebral artery occlusion (tMCAO) mouse model were constructed. Results: circCELF1 was found to be upregulated in OGD/R-induced astrocytes, relative to normal astrocytes. circCELF1 knockdown repressed the apoptosis and autophagy of astrocytes. The in vivo study conducted with the tMCAO model also revealed that circCELF1 or NFAT5 deficiency contributed to the suppression of neural injury. Further, circCELF1 was uncovered to elevate NFAT5 expression via recruiting DDX54, functionally promoting astrocyte apoptosis and autophagy. Conclusion: circCELF1 recruits DDX54 to upregulate NFAT5, by which astrocyte apoptosis and autophagy are stimulated.

Background Umbilical cord blood acid–base sampling is routinely performed at many hospitals. Recent studies have questioned this practice and the association of acidosis with cerebral palsy. Objective To investigate the associations between the results of umbilical cord blood acid–base analysis at birth and long‐term neurodevelopmental outcomes and mortality in children. Search strategy We searched six databases using the search strategy: umbilical cord AND outcomes. Selection criteria Randomised controlled trials, cohorts and case–control studies from high‐income countries that investigated the association between umbilical cord blood analysis and neurodevelopmental outcomes and mortality from 1 year after birth in children born at term. Data collection and analysis We critically assessed the included studies, extracted data and conducted meta‐analyses comparing adverse outcomes between children with and without acidosis, and the mean proportions of adverse outcomes. The certainty of evidence was assessed using the Grading of Recommendations, Assessment, Development and Evaluations approach. Main results We have very low confidence in the following findings: acidosis was associated with higher cognitive development scores compared with non‐acidosis (mean difference 5.18, 95% CI 0.84–9.52; n = two studies). Children with acidosis also showed a tendency towards higher risk of death (relative risk [RR] 5.72, 95% CI 0.90–36.27; n = four studies) and CP (RR 3.40, 95% CI 0.86–13.39; n = four studies), although this was not statistically significant. The proportion of children with CP was 2.39/1000 across the studies, assessed as high certainty evidence. Conclusion Due to low certainty of evidence, the associations between umbilical cord blood gas analysis at delivery and long‐term neurodevelopmental outcomes in children remains unclear.
