“…This approach is considered to be a safer surgical technique than transposition procedures because it involves the manipulation of fewer extra-ocular muscles, with less risk of anterior segment ischemia. Furthermore, it is well known that the IOAT and myectomy procedure has been effectively used for the treatment of IO overaction, with or without DVD, and, in selected patients with unilateral fourth nerve palsy and VD [4][5][6][7]. Although Gonzales and Klein [4] reported that myectomy and IOAT is an easier and effective alternative operation to treat IO overaction, in the present case, we performed a 6-mm myectomy of the IO only because of abnormal muscular flaccidity, which was attributed to the trauma.…”
