Trends of Incidence, Clinical Presentation, and In-Hospital Mortality Among Women With Acute Myocardial Infarction With or Without Spontaneous Coronary Artery Dissection

Abstract: Women presenting with AMI and SCAD appear to be at higher risk of in-hospital mortality. Lower rates of PCI were associated with improved survival, with evidence of worse outcomes when PCI was performed for SCAD in the setting of non with ST-segment elevation myocardial infarction.

“…However, a strong suspicion should be raised in young women presenting with MI as close to 25% of women <50 years old with MI have SCAD on angiography. 15 Although the prevalence of SCAD has been estimated to be between 0.07% and 1.1% of all coronary angiographies performed, 3,16 the exact prevalence is unknown as SCAD can be difficult to diagnose secondary to dissections that may mimic atherosclerotic plaque. Intracoronary imaging, which includes intravascular ultrasound and optical coherence tomography, has aided the accurate diagnosis of SCAD at the expense of an increased risk of extending the dissection.…”

“…The NIS was developed for the Health Care Cost and Utilization Project (HCUP) [11]. The NIS has been validated internally and externally [12,13]. It has been used previously for describing trends and outcomes of various diseases [14,15].…”

“…SCAD is a rare cause of ACS, but 90% of the cases are reported in women (table 2). 28 In large registries, SCAD is reported as the cause of acute myocardial infarction (AMI) in 1% of cases 29. This is most probably underestimated due to the diagnosis done by standard coronary angiography which cannot detect the dissection if it is only present as intramural haematoma occluding the lumen in the absence of dye fixation typical of intimal tear.…”
