Treatment of Pemphigus With Gold

Abstract: Background: Although gold has been reported to be useful in treating pemphigus vulgaris, its use has waned in recent years because of concerns regarding efficacy and toxicity. Objective: To review 26 patients with pemphigus who were treated with intramuscular gold over a 10-year period. Results: Gold was effective in 62% of patients as a primary treatment for pemphigus or as a steroid-sparing agent. “…A study reported in 1998 by Pandya and Dyke reviewed the efficacy of gold in the treatment of 26 patients with pemphigus (21 of whom had PV). 48 Eighty-five percent of the patients had responses; however, only 4 (15%) were in complete remission at the end of the study. Seven patients were able to discontinue prednisone and continue in remission while receiving gold alone, while 5 patients had responses to gold but had some disease activity by the end of the study, even while taking prednisone and gold.…”

“…All complications resolved after discontinuation of the gold. 48 The efficacy of gold is generally inferior to that of immunosuppressive drugs. However, the use of gold may be preferable in patients in their reproductive years, as it lacks the carcinogenic and infertility effects of most of the other immunosuppressive drugs.…”

“…Die hier vorgestellten Fälle und die Berichte aus der Literatur [4,20,22] zeigen, dass darüber hinaus die Behandlung des PV mit Gold (Na-aurothiomalat) als eine sehr wirksame Therapieoption, insbesondere bei refraktären Schleimhautläsionen, zu werten ist. Nach unseren Erfahrungen gilt dies zusätzlich für Fälle eines lokalisierten Pemphigus (unveröffentlichte Daten).…”
