Résumé -Introduction : La chéilite granulomateuse de Miescher (CGM) est une tuméfaction des lèvres primaire ou secondaire à une pathologie sous-jacente. L'objectif de cet article est de montrer la difficulté du bilan étiologique et les différents traitements disponibles. Observation : Une femme de 77 ans a consulté pour une tuméfaction chronique isolée de la lèvre inférieure. La biopsie a retrouvé de multiples foyers granulomateux épithélioïdes et gigantocellulaires, sans nécrose caséeuse. Le bilan étiologique a été refusé par la patiente. Un traitement par injection intra-lésionnelle de triamcinolone acétonide 40 mg retard a été instauré avec succès. Discussion : La CGM peut être primaire ou secondaire. Le bilan étiologique est indispensable. Le traitement est rapporté comme un défi.Abstract -Granulomatous cheilitis of Miescher: diagnostic difficulty. Introduction: Granulomatous cheilitis of Miescher (GCM) is a swelling of the lips primary or secondary to an underlying disease. The objective of this article is to report the difficulty of etiological diagnosis and various treatments available. Observation: A 77 years woman has consulted for a chronic swelling of the lower lip. Biopsy found multiple epithelioid granulomatous foci with giant cells without caseous necrosis. The etiological diagnosis was refused by the patient. Treatment with intralesional injection of triamcinolone acetonide 40 mg was done successfully. Discussion: GCM can be primary or secondary. L'interrogatoire a retrouvé une fibrillation auriculaire traitée par fluindione. L'examen clinique a révélé une tuméfaction généralisée de la lèvre inférieure associée à une desquamation (Fig. 1). La chronicité de la lésion, sans épisode de régression, a permis d'éli-miner un oedème de Quincke ou un oedème angioneurotique. Il n'existait aucune autre lésion muqueuse, pas de fissure linguale ni d'antécédents de paralysie faciale. Une biopsie a été réalisée. L'examen histopathologique a retrouvé de multiples foyers granulomateux épithélioïdes et giganto-cellulaires, sans nécrose caséeuse centrale (Fig. 2). Il existait un infiltrat lymphocytaire péri-granulomateux, sans vascularite. La coloration de Ziehl-Neelsen était négative ce qui permettait d'éli-miner une infection à mycobactéries. La numération sanguine et la VS étaient normales. La patiente a refusé toutes les investigations proposées pour éliminer une MC ou une sarcoïdose. Cheilitis granulomatosa (CG) is a rare disease, which presents usually as a persistent swelling of the soft tissues in the orofacial region and is characterized histologically by a granulomatous inflammation. We report the case of a 19-year-old man who suffered from anal fistula. The patient had a 6-year history of asymptomatic and persistent swelling of the lower lip. Examinations for gastrointestinal lesions containing double-balloon total enteroscopy revealed erosions located longitudinally throughout the small intestine and the patient was diagnosed Crohn's disease (CD). Biopsy of the lower lip showed non-caseating granuloma and confirmed the diagnosis of CG. Despite an elemental diet and mesalazine therapy, the lip swelling persisted. The CG can be the first presenting symptom of CD. CG as a complication of CD is discussed.
