Abstract: The incidence of clinically significant anastomotic leaks after upper gastrointestinal surgery is approximately 4 % - 20 %, and the associated mortality can be as high as 80 %. Depending on the clinical presentation, the treatment options are surgery, conservative treatment with external drainage, or endoscopic treatment. This report presents 39 cases of clinically apparent anastomotic leaks or fistulas after surgery for upper gastrointestinal cancers that were treated by endoscopy with insertion of fibrin glu… Show more

“…Larger leaks and fistulae have been closed with good long term success using Vicryl mesh (Ethicon, Hamburg, Germany) combined with fibrin sealant. 87 The mesh is placed over the defect and covered with 2-3 A B C mL fibrin (Tissucol Duo, Baxter, Germany) and fibrin is injected on the edges which eventually epithelializes with time. Rábago et al 88 reported a high success rate of 86.6% closure of fistulae with sealant after failure of conservative treatment at a favorable cost; however, only 55% of high-output fistulae achieved complete closure of defect.…”

“…87 Vicryl mesh is made of Polyglactin 920 (90% glycolide and 10% L-lactide) with pores large enough to readily absorb fibrin glue. The mesh is placed over the defect and covered with 2-3 mL of fibrin and fibrin is injected into the surrounding submucosal layer which eventually epithelialize.…”

“…Treatment success with further endoscopic procedures was seen. Vicryl plug or clipping are reported as additional endoscopic procedures because endoscopic treatment failure with consecutive surgical intervention became necessary [15,[19][20][21][22].…”

“…Each route of transposition has advantages, but are often under debate, since the posterior route is shorter than the anterior route, but mediastinal leakage is more dangerous than leakage on the neck. Anastomotic leakage is a feared complication of reconstruction with an incidence of 3-30% and a mortality rate of 25-50% (2)(3)(4)(5)(6)(7).…”
