Traumatic intratumoral hemorrhage as the presenting symptom of a spinal neurinoma

Abstract: ✓ Intratumoral hemorrhage as the presenting symptom of spinal tumors is rare. The authors describe a patient who presented with rapidly progressing paraplegia 24 hours after sustaining a minor traumatic injury of the thoracic spine. Radiological evaluation demonstrated a low-thoracic intradural tumor that was resected and found to be a neurinoma in which severe intra- and peritumoral hemorrhage was revealed. The radiological, surgical, and pathological findings are presented a… Show more

“…12 Intratumoral hemorrhage in spinal schwannoma is a rare occurrence. 3,8,15 Ichinose et al 8 reviewed only 3 cases of solitary intratumoral hemorrhage, cases that presented exclusively with spinal symptoms and neurological deficit in the lower limbs. In our case, there was clinico-radiological and histological evidence of intratumoral hemorrhage with simultaneous intracranial SAH, which, to the best of our knowledge, has not been previously described.…”

“…[4][5][6][7] Similarly few reports have described hemorrhagic conversion of spinal schwannomas presenting with spinal hematomas and related symptoms. [8][9][10][11][12][13][14][15][16] These reports have described hemorrhage from spinal schwanno- Table 1 and Figure 2 show a distribution in the spine of spinal schwannomas presenting with intradural subdural/ intratumoral hemorrhage, with most frequent presentation in the thoracic spine. Several reports have postulated changes in vascular supply as contributing factors to hemorrhage development in the setting of spinal schwannomas.…”

“…Cohen et al describes the case of a patient presenting with acute paraplegia following a minor traumatic thoracic spine injury and was found to have a hemorrhagic intradural nerve sheath tumor. 13 In a case report by Uemura et al, the authors describe the case of a woman presenting with acute paraparesis following physical exertion due to hemorrhagic spinal schwannoma.…”

“…Presenting symptoms are most commonly progressive lower back pain, numbness or paresthesias, neurological deficits, and failure of conservative therapy such as physical rehabilitation [10]. However, acute lower back pain may also be caused nerve root torsion or hemorrhage within the tumor itself [11,12]. Nonspecific abdominal pain has also been reported [13].…”
