Myofunctional therapy (MFT) may have a role in improving muscle tone and alleviating upper airway collapse in sleep-disordered breathing. The purposes of this state-of-the-art review are to first review systematically the current literature on the effectiveness of MFT in treating sleep-disordered breathing and then to provide an overview of the current understanding of patient selection, side effects, type and duration of exercises, guidance of exercise performance, evaluation of results, and how best to promote adherence. PubMed (Medline), the Cochrane Library, and the EMBASE, Scopus and SciELO databases were checked for relevant studies by three authors, and a total of 23 studies were included. This review focuses only on adults with sleep-disordered breathing. The available evidence shows a positive effect of MFT in reducing sleep apnoea, as measured using polysomnography and clinical variables (including snoring). There is no evidence of the utility of MFT for treating upper airway resistance syndrome, the duration of the effects of MFT, or regarding which MFT protocol is best. Despite these knowledge gaps, the available evidence suggests that MFT is a safe treatment modality.

ObjectiveThere are no official diagnostic tools to evaluate the weakness of the genioglossus muscle. We have developed a protocol for muscular assessment in patients with severe obstructive sleep apnea‐hypopnea syndrome (OSAHS) and evaluated its effectiveness.Study DesignCase and controls prospective study.SettingSleep Unit Hospital Quironsalud Marbella (Spain).MethodsTwenty‐nine cases and 20 controls were recruited. Patients were examined by a phonoaudiologist that performed biometric measurements and the Orofacial Myofunctional Evaluation With Scores (OMES), Friedman, and Epworth Sleepiness Scale (ESS). In addition, upper airway muscle strength measures were performed using the Iowa Oral Performance Instrument (IOPI) and Tongue Digital Spoon (TDS).ResultsThe final cohort consisted of 49 subjects, including 29 cases and 20 controls. According to the univariate and multivariate logistic regression analyses, ESS, OMES protocol, IOPI score, and TDS were associated with severe OSAHS. Multivariate regression revealed an IOPI score below 48 kps with an adjusted odds ratio (OR) of 9.96 (95% confidence interval [CI] 2.5‐39.1, p = .001), and a 0.72 specificity (Spe), a 0.79 sensitivity (Sens), and a 0.82 area under the curve (AUC). Similarly, an OMES score lower than 200 had an adjusted risk ratio of 4.02 (95% CI 2‐7, p < .001), 1 Spe, 0.79 Sens, and 0.98 AUC; and finally, TDS scores lower than 201 g/cm2 showed an adjusted OR of 27 (95% CI 4.74‐153.6, p = .0001), 0.66 Spe, a 0.93 Sens, and a 0.86 AUC.ConclusionOur findings suggest that severe OSAHS patients present different muscle patterns than controls.

El objetivo de este trabajo de revisión fue identificar los principales tratamientos disponibles para el síndrome de apnea obstructiva del sueño. Se realizó una búsqueda de información en las bases de datos Pubmed, BVS, Science Direct, Scopus, Web of Science, Scielo, Ovid, Lilacs, Research Gate y Redalyc usando los distintos criterios de exclusión e inclusión. Se realizó la valoración cualitativa con un total de 22 artículos. De estos 7 Ensayos Clínicos Aleatorizados, 1 Revisión Sistemática, 2 Informe de Casos, 2 Meta-Análisis, 1 Estudio Piloto Retrospectivo y 9 Revisiones bibliográficas. Existen varios tratamientos enfocados a resolver el síndrome de apnea obstructiva del sueño, entre los principales tenemos: la terapia miofuncional, el tratamiento quirúrgico, los distintos aparatos o dispositivos orales, la presión aérea continua, el tratamiento farmacológico, la estimulación del nervio hipogloso, la pérdida de peso y el tratamiento médico. El Gold standard es la presión positiva aérea continua. Sin embargo, los tratamientos deben ser aplicados según el tipo de problema (leve, moderado o severo) y en muchos casos el tratamiento puede ser combinado para obtener mejores resultados.
