Tratamento híbrido com endoprótese não recoberta nas dissecções agudas da aorta tipo A

Abstract: The new surgical strategy to treat patients with acute type A aortic dissection, the hybrid procedure with an uncovered aortic stent, allows surgeons to treat the aortic arch and the proximal descending aorta, besides the ascending segment, without extension of cerebral or systemic ischemia.

“…The need for surgical treatment immediately after diagnosis aims at preventing its life-threatening complications, such as cardiac tamponade secondary to rupture of the aorta, as reported in this case, acute myocardial infarction, symptomatic aortic insufficiency, neurological complications, among others, with a dramatic impact on mortality 15,16 . The gold standard treatment is the surgical intervention and the traditionally proposed surgery is the interposition of the supracoronary tube, of which in-hospital mortality rate varies, ranging between 10 and 30% 17,18 . Unfortunately, this patient did not survive long enough to undergo such treatment.…”

“…The proposal of employing hybrid procedures in the treatment of acute aortic dissections 39 and the study of the use of evoked potentials in the monitoring of medullary distress during the surgical approach of the descending aorta 40 have opened up new perspectives in the treatment of thoracic aorta disorders. On the other hand, the Brazilian experience regarding the use of mechanical circulatory assistance devices includes only initial series with extracorporeal membrane oxygenation (ECMO) 41,42 and paracorporeal artificial ventricles in the treatment of cardiogenic shock or as bridge to heart transplant 43 .…”
