Transmaxillary-Transnasal Approach to the Anterior Clivus

Abstract: Numerous procedures to expose the anterior clival region have been described, including the transoral, transcervical, transseptal-transsphenoidal, transantral, transnasal, bilateral Le Fort I maxillotomy, transbasal, transpalatal, and modifications of the Caldwell-Luc approach. Despite the large number of surgical options available, it may be necessary to have wider access to the midline skull base than these approaches provide. We have developed a microsurgical transmaxillary-transnasal approach to the anteri… Show more

“…Anterior approaches to the cervical vertebral junction and clivus were originally proposed for the treatment of vertebrobasilar aneurysms [22,23] or neoplasms [24]. Anterior access route includes the transseptal-transsphenoidal, transmaxillary, transpalatal and transoral-transpalatal, transmandibular, transmaxillary-transnasal, and the facial translocation approaches [25][26][27][28][29][30][31]. And now the endoscopic approach to the clivus has become a well-established alternative to traditional surgery.…”

“…When a lateral extension is needed, a transmaxillary route may be used [40][41][42][43]. Endoscopy may maximize the exposure in all directions [44][45][46][47].…”

“…A detailed analysis of the advantages and disadvantages of each approach proposed for resection of skull base chordomas is beyond the scope of this article, and excellent reports have already been published on this topic. 1,11,17,23,26,27,33,36,37,45,47,51,55,57,58,76,[86][87][88][89]94 Careful study of the preoperative neuroradiological studies is essential when planning the surgical approach to these challenging lesions. In recent years, continuous refinements of frameless stereotactic systems have come to provide the surgeon with intraoperative feedback regarding surgical orientation.…”
