Objective: The purpose of this study is to review the main postpartum screening tools currently used in terms of their ability to screen for postnatal depression. Furthermore, the cultural characteristics of depressive postpartum symptomatology are examined. Method: A systematic literature search was conducted for the period 1987-2009, using the Medline electronic database for the following keywords: postpartum depression and postnatal depression. These terms were combined with: assessment, screening and psychometric tools. Results: Of the four screening tools reviewed and compared, the Edinburgh Postnatal Depression Scale (EPDS) and the Postpartum Depression Screening Scale (PDSS) presented substantial sensitivity and specificity as screening tools. However, none of the instruments could be rated flawless when applied to different cultural contexts. Conclusions: In addition to the EPDS, a new generation of instruments is currently available. Supplementary research is needed to substantiate their use as screening tools in general practice. Additional studies are needed to adapt and test instruments to detect postnatal depression within a wider range of languages and cultures.

In this paper we review some of the best available evidence to argue that screening for perinatal depression should be systematically conducted since pregnancy. Our view is organized in ten topics: (1) perinatal depression high prevalence; (2) its potential negative consequences, including maternal, conjugal, foetal, infantile, and child effects; (3) its under-detection and treatment; (4) its stigma; (5) the professionals and women misconceptions related to perinatal depression; (6) the availability of valid and short self-report screening instruments for perinatal depression and (7) their acceptability; (8) the increase in recognition, diagnosis, and treatment rates in comparison with routine practice; (9) the opportunity, given the large number of contacts that women have with health professionals in the perinatal period; and (10) perinatal depression screening potential cost-effectiveness.

ResumoObjetivo: O pós-parto é um período de alterações biológicas, psicológicas e sociais. Essa é considerada a época mais vulnerável para a ocorrência de transtornos psiquiátricos. A disforia puerperal, a depressão pós-parto e a psicose pós-parto têm sido classicamente relacionadas ao pós-parto. Atualmente, tem sido observado que os transtornos ansiosos também estão associados a esse período. Método: Neste artigo é feita uma revisão da bibliografia acerca de transtornos psiquiátricos no pós-parto a partir de artigos encontrados no PubMed e no SciELO entre os anos de 2000 e 2009. Livros, teses e outros artigos considerados relevantes citados no material consultado também foram incluídos. Resultados: A disforia puerperal ocorre em 50% a 85% das mulheres, o quadro é leve e transitório e não requer tratamento. A depressão pós-parto tem prevalência em torno de 13%, pode causar repercussões negativas na interação mãe-bebê e em outros aspectos da vida da mulher e deve ser tratada. A psicose pós-parto é rara, aparecendo em cerca de 0,2% das puérperas. Tem quadro grave que envolve sintomas psicóticos e afetivos, havendo risco de suicídio e infanticídio e geralmente requerendo internação hospitalar. Os transtornos ansiosos podem ser exacerbados ou precipitados no pós-parto, especialmente o transtorno de ansiedade generalizada, o transtorno de estresse pós-traumático e o transtorno obsessivo-compulsivo. Conclusão: Apesar de não serem reconhecidos como entidades diagnósticas pelos sistemas classificatórios atuais, os transtornos mentais no puerpério apresentam peculiaridades clínicas que merecem atenção por parte de clínicos e pesquisadores.Cantilino A, et al. / Rev Psiq Clín. 2010;37(6):278-84 Palavras-chave: Transtornos psiquiátricos, pós-parto, depressão, ansiedade, psicose. Puerperal blues, postpartum depression and postpartum psychosis have been classically associated to the postpartum. Anxiety disorders have also recently been associated to this period.
