Transition: the cancer perspective

Abstract: There is an ever-increasing number of cancer survivors entering adult life. Health-care professionals in this field must ensure that there is seamless transition through paediatric, teenage and young adult and adult services such that an individual patient is able to achieve his/her maximum potential and enjoy a quality, 'cancer-free' future.
