Abstract: We present the case of a 41-year old female who developed a complete facial nerve palsy after an interhemispheric approach for clipping of a distal anterior cerebral artery aneurysm. Work-up revealed that she had developed acute parotitis during surgery, possibly from obstruction of the parotid duct by the tracheal tube.
