BACKGROUND Existing cellulite interventions pose various clinical challenges related mostly to ecchymosis and recovery time. OBJECTIVE To discuss the current treatment options for minimizing recovery time, efficacy of these options, and investigations into possible future approaches. METHODS A roundtable meeting was convened to discuss and share views on the clinical challenges seen in the present practice of cellulite treatments along with future approaches and mitigation strategies. The participants' views helped guide a narrative review on this topic. RESULTS Cosmetic clinicians have a range of new interventions to choose from for cellulite improvement, each with different benefits and safety aspects. Bruising is a typical side effect that is seen with treatments targeting the fibrous septa, such as subcision and injectable treatments, and in some cases may produce long-lasting hyperpigmentation from postinflammatory hyperpigmentation or hemosiderin staining. Various strategies that could potentially mitigate bruising and other adverse effects of cellulite treatment are under clinical investigation, including, but not limited to, different injection techniques and dilutions, compression garments, cold packs, arnica gel, pulsed dye laser treatment, intralesional epinephrine, and tranexamic acid. CONCLUSION Clinical challenges including varying treatment outcomes and certain treatment sequelae remain, and further research is needed to prevent side effects and improve treatment outcomes.

Die postpartale Hämorrhagie tritt bei etwa 4 % aller Entbindungen in industrialisierten Ländern auf und zeigt eine steigende Inzidenz. Die schwerwiegenden Auswirkungen auf das maternale Outcome erfordern eine rasche Diagnosestellung und Ursachenidentifizierung mit nachfolgendem zielgerichtetem sowie interdisziplinärem Management der Blutstillung, Kreislaufstabilisierung und Gerinnungstherapie. Epidemiologie Die Reduktion der Müttersterblichkeit wurde von den Vereinten Nationen und der World Health Organization (WHO) als eines der globalen Ziele für nachhaltige Entwicklung deklariert. Dennoch verstarben im Jahr 2020 weltweit etwa 270 000 Frauen an den Folgen von Schwangerschaft und Geburt, davon 95 % in Drittwelt-und Schwellenländern. Nachdem die Rate mütterlicher Sterbefälle in den letzten 20 Jahren um etwa 34 % gesunken ist, veröffentlichte die WHO nun eine Stagnation dieser Entwicklungund sie berichtete einen signifikanten Wiederanstieg der Mortalitätsraten in Europa und Nordamerika seit 2016 [1].Mit etwa 27 % aller Todesfälle weltweit und bis zu 16 % in westlichen Ländern gehört die postpartale Hämorrhagie (PPH) dabei zu den Hauptursachen maternaler Mortalität [2]. Die PPH betrifft bis zu 4% aller Entbindungen, die schwere PPH etwa 0,3% mit jeweils steigender Inzidenz. Diese Entwicklung ist dabei auch auf die wachsende Anzahl mütterlicher Risikofaktoren und die steigende Sectiorate mit konsekutiven Folgekomplikationen wie Plazentationsstörungen und Uterusnarben zurückzuführen [3,4,5].

Purpose of reviewTraumatic brain injury (TBI) is a leading cause of trauma-related deaths, and pharmacologic interventions to limit intracranial bleeding should improve outcomes. Tranexamic acid reduces mortality in injured patients with major systemic bleeding, but the effects of antifibrinolytic drugs on outcomes after TBI are less clear. We therefore summarize recent evidence to guide clinicians on when (not) to use antifibrinolytic drugs in TBI patients. Recent findingsTranexamic acid is the only antifibrinolytic drug that has been studied in patients with TBI. Several recent studies failed to conclusively demonstrate a benefit on survival or neurologic outcome. A large trial with more than 12 000 patients found no significant effect of tranexamic acid on head-injury related death, allcause mortality or disability across the overall study population, but observed benefit in patients with mild to moderate TBI. Observational evidence signals potential harm in patients with isolated severe TBI.
