TRALI due to granulocyte‐agglutinating human neutrophil antigen‐3a (5b) alloantibodies in donor plasma: a report of 2 fatalities

Abstract: It is concluded that female blood donors with only two previous pregnancies can form clinically important granulocyte-reactive alloantibodies leading to fatal TRALI reactions in recipients. The sometimes devastating consequences of TRALI should prompt the development of strategies to prevent or reduce its incidence. Further research is warranted to investigate recipient and donor factors responsible for TRALI, including whether 5b (HNA-3a) alloantibodies are especially prone to cause severe reactions, and to b… Show more

“…Because anti-HNA-3a antibodies cause the most severe TRALI, 3,18,19 we tested whether such antibodies could promote NET formation by human neutrophils. IgG were purified from 2 blood donors with anti-HNA-3a antibodies whose plasma caused a TRALI reaction in patients and control IgG were purified from a control donor's plasma.…”

“…1,4,[11][12][13][14] Although a number of neutrophil-specific antibodies have been reported, the most common is directed at the 5b (HNA3a) antigen. 6,15 In a small percent of cases, the leukoagglutinating antibody appears to be from the recipient and is directed at the transfused neutrophils. [16][17][18] Most of the donors implicated in TRALI have been multiparous women who became alloimmunized during pregnancy.…”

“…Blood components with a high plasma fraction (typically fresh-frozen plasma, apheresis platelets and whole blood) should not be prepared from blood donated by donors with leucocyte antibodies. In the case of HNA-3a antibodies, deferral of the donor from blood donation should be considered, as these antibodies have been frequently implicated in fatal TRALI reactions [12,16] and one fatal TRALI reaction occurred even after transfusion of red blood cells [12]. Donors with negative test results for leucocyte antibodies should be retested only after re-exposure to leucocyte antigens.…”
