The authors report a case of a common bile duct cyst in a 19-year-old patient referred for the management of chronic pain in the right hypochondrium, without other associated symptoms. The abdominal computed tomography scan revealed a bile duct cyst confirmed by abdominal magnetic resonance imaging (MRI) (type 1c according to Todani). Laparotomy had revealed a common bile duct cyst. The procedures consisted of a resection of the main bile duct and cholecystectomy + hepatico-jejunal anastomosis (Roux-en-Y method). The anatomopathological examination had shown a fibrous-walled bile duct cyst with epithelial hyperplasia and dysplasia without signs of malignancy. The post-operative follow-up was simple and the return home had taken place on the 7 th postoperative day. The patient had been seen for routine check-up 51 days after surgery. She was in apparent good health without particular complaints.

Además de la reconstrucción del tránsito biliar, que se consigue mediante la anastomosis bilio-digestiva en "Y de Roux" (anastomosis hepaticoyeyunal), se considera que la escisión es completa cuando conlleva la resección de la totalidad de la vía biliar extrahepática incluido el colédoco intrapancreático, esto permite suprimir de forma definitiva el reflujo pancreático y con ello el riesgo de malignización.…”

INTRODUCCIÓN:El quiste del colédoco es una malformación congénita de etiología desconocida, caracterizada por diversos grados de dilatación del sistema biliar. El examen inicial para la valoración de la vía biliar debe ser una ecografía abdominal,sin embargo, la resonancia magnética con contraste es el estudio de elección para valorar estas estructuras. El tratamiento quirúrgico final depende del grado de dilatación quística y generalmente está basado en la escisión completa de la vía biliar extrahepática.CASO CLÍNICO: Paciente de sexo femenino de 27 años con dolor abdominal tipo cólico a nivel de epigastrio e hipocondrio derecho de 7 días de evolución. Se realizaron exámenes de Imagenología (ecografía-TAC -colangio RM)que reportaron colédoco con marcada dilatación fusiforme en toda su extensión, en relación a quiste de colédoco tipo IA.EVOLUCIÓN: Se planificó la exéresis del quiste de colédoco, anastomosis bilio-digestiva en «Y de Roux» y colecistectomía. El procedimiento quirúrgico no tuvo complicaciones. La paciente permaneció hospitalizada por 5 días con adecuada evolución y tolerancia a dieta. CONCLUSIÓN:El quiste de colédoco es una patología congénita infrecuente en la edad adulta generalmente diagnosticada como un hallazgo incidental. El diagnóstico y tratamiento oportuno aseguran el éxito del procedimiento quirúrgico y evita la aparición de complicaciones graves (colangiocarcinoma); con la escisión completa el pronóstico es excelente, la mortalidad posoperatoria es mínima. *DESCRIPTORES DeCS

We present a clinical case (new born) of female at 11 months of life for an abdominal mass. In this context, we discuss the surgical procedure step by step. Cystic dilatation of the common bile duct can be defined as pseudo aneurysmal congenital ectasia biliary tract, a rare condition first described in 1723 by VATER. In the light of this presentation and after a review of the literature, we highlight the rarity of cystic dilation of the bile duct. It may be misunderstood. The diagnosis is most often only carried out intra operatively. Left untreated, the condition can lead to secondary biliary cirrhosis. The treatment is surgical and the current trend is unanimous: the radicality of the gesture by resection of the cyst when it is possible with hepatico-jejunal anastomosis.
