This investigation was aimed at assessing a possible interaction of a traditional Thai herbal recipe, “Ya-Sa-Marn-Phlae (YSMP),” used for wound treatments with topical antiseptics, povidone-iodine (PI) solution and hydrogen peroxide (H2O2), and effects of THR-SK010 alone and the combinations on Staphylococcus aureus. Antibacterial activities of ethanol extracts from the herbal recipe were determined against both methicillin resistant Staphylococcus aureus (MRSA) and methicillin-susceptible S. aureus (MSSA). YSMP exhibited remarkable antistaphylococcal activity with MIC values of 3.9–7.8 μg/mL. This recipe possessed bacteriostatic activity and did not reduce the tolerance of both MRSA and MSSA isolates to the high ionic strength. Interaction between THR-SK010 and the antiseptics was carried out by checkerboard testing and time-kill assay. Both indifferent and slightly antagonistic effects were observed with THR-SK010/PI and THR-SK010/H2O2 combinations against the tested isolates. In addition to commercially available antiseptics, THR-SK010 offered additional therapeutic options for the decolonization of MRSA and MSSA. Topical application of plant extracts with antioxidant activity, such as THR-SK010, should not be used immediately with PI or H2O2 and further investigation on this interaction is needed.

In this work, theospheres (innovative lipid nanoparticles) were prepared by the high pressure homogenization technique using different surfactants for dapsone encapsulation. Mean particle size ranged from 105 to 153 nm and negative zeta potentials were obtained for all theosphere formulations. Atomic force microscopy images confirmed the spherical shape of theospheres. The HPLC method used to determine dapsone-loaded theospheres was selective, linear, exact and precise. The entrapment efficiency of dapsone was 91.4%. Theospheres provided controlled release of idebenone (52.7 ± 1.6%) in comparison to the free drug (103.1 ± 1.9%).Keywords: lipid nanoparticles; dapsone; in vitro release study. INTRODUÇÃOA dapsona (4-4'diaminofenilsulfona) (Figura 1) é uma sulfona que possui dupla atividade -antimicrobiana e anti-inflamatória/imunomoduladora. Devido a esta dupla ação, a dapsona é utilizada como terapia única ou em conjunto com outros fármacos no tratamento de doenças provocadas por patógenos (a exemplo de hanseníase e pneumonia por Pneumocystis jiroveci) e doenças inflamatórias crô-nicas, especialmente as dermatológicas.1 Adicionalmente, dapsona tem demonstrado ser eficiente na inibição de espécies reativas de oxigênio (ROS), mostrando-se tão eficaz quanto catalase na redução dos níveis de H 2 O 2 e tão potente quanto colchicina, benzoato e xantina na redução dos níveis de OH -em ensaios in vitro. 2Quando administrada por via oral, dapsona é metabolizada originando hidroxilaminas, responsáveis por efeitos adversos importantes como hemólise e metemoglobinemia. A hemólise caracteriza-se pela ruptura da membrana plasmática das hemácias levando ao rompimento destas células, enquanto a metemoglobinemia é uma condição em que a hemoglobina perde a capacidade de se ligar ao oxigênio e transportá-lo.3 Ambas as condições são observadas em todos os pacientes tratados com dapsona por via oral. Além disso, a administração sistêmica de dapsona reduz o tempo de meia-vida dos eritrócitos e pode levar à hemólise grave em pacientes que apresentam deficiência na enzima glicose-6-fosfato desidrogenase. Outros efeitos colaterais menos frequentes observados com este tratamento incluem reações idiossincráticas como agranulocitose, erupção cutânea, leucopenia, neuropatia periférica, além de sintomas neurológicos e gastrointestinais. 4 Estudos já demonstraram equivalência na atividade da dapsona aplicada topicamente (0,1; 0,5; 1; 5 e 10%) e sistemicamente (100 mg/ dia) na supressão de eritema cutâneo causado por radiação UVB em voluntários saudáveis.2 Assim, uma abordagem para reduzir os efeitos colaterais é a administração de dapsona diretamente na área afetada.
