Time for Paradigm Change in Management of Hepatocellular Carcinoma: Is a Personalized Approach on the Horizon?

Abstract: Hepatocellular carcinoma (HCC) is the fifth most frequent cancer form but the second leading cause of all cancer-related deaths. There are several reasons for high mortality in the HCC cohort: lack of effective screening programs and consequently late diagnosis, multifactorial origin with cumulative risk factors, complex carcinogenesis, tumor heterogeneity, unpredictable impacts of individual microenvironment on tumor development and progression, and, as the consequence, frequently untargeted therapy and cance… Show more

“…Practical implementation of novel and complex laboratory tests certainly should be considered from the viewpoint of their reasonability, cost-effectiveness and value added to a data interpretation. Smart laboratory investigation strategies and all-encompassing data interpretation are essential for an appropriate relationship between laboratory medicine and clinicians acting hand-in-hand as the decision makers responsible for better individual outcomes [7075]. …”

“…MMPs, especially MMP‐2 and MMP‐9, are highly expressed in pathological tissues derived from patients with metastatic HCC after SIRT treatment . In recent years, patients with metastatic HCC have been treated with personalized management and tailored therapy . GP73 might be a potential target in tailored therapy targeting metastatic HCC.…”

“…Hepatocellular carcinoma (HCC) is ranked as the fifth most common cancer but the second leading cause of all cancer-related mortalities [ 159 ]. This has been due to lack of effective screening programs and consequently late diagnosis, multifactorial origin with cumulative risk factors, complex carcinogenesis, tumour heterogeneity, unpredictable impacts of individual microenvironment on tumour development and progression, and, as the consequence, frequently untargeted therapy and cancer resistance towards currently applied treatment approaches [ 159 ]. Currently the “wait and treat” approach is inappropriate in the overall hepatocellular carcinoma management.…”
