Three-dimensional Computed Tomography for Analyzing the Vascular Anatomy in Laparoscopic Surgery for Right-sided Colon Cancer

Abstract: Our study clarified the anatomic variety of the vessels in right-sided colon cancer. Preoperative 3D-CT is useful for understanding the anatomy to ensure a safe, precise operation.

“…This software offers a standardized analysis of 3D anatomy based on two-dimensional MDCT imaging. A preoperative conference based on the resulting 3D images allowed for the interpretation of 3D anatomical images from any laparoscopic view and the sharing of these anatomical images with surgical staff [ 1 3 ]. Furthermore, during the actual surgery, the surgical staff was able to view the preoperative 3D images on a large display, which facilitated discussion of the vessel arrangement pattern.…”

“…Laparoscopic dissection of lymph nodes while taking into account the aforementioned vessel arrangement is technically demanding and time consuming [ 3 , 4 ]. One reason for this is that several vessel arrangement patterns exist among the ICA, ICV, RCA, GCT, and SMV.…”

“…To date, multidetector row CT (MDCT) angiography has been widely used for objective vascular morphological analyses . The modality is sometimes used to assess the possible strictures in the anastomosed arteries of the implanted patients under unstable renal function.…”

“…The assessment of intrarenal arterial morphology is important in transplant renal arteries, where vascular complications such as anastomotic stenosis lead to unfavorable outcomes . Computed tomography angiography (CTA) and digital subtraction angiography (DSA) are commonly used methods for the depiction of intrarenal arteries, and have been routinely performed in examination of living donors; however, these methods may be suboptimal for recipients, because they require iodinated contrast media that is potentially nephrotoxic. Gadolinium chelate does not jeopardize renal function in recipients; however, there is a risk of nephrogenic systemic fibrosis (NSF) .…”

“…Nevertheless, the disadvantages to this approach include lack of a panoramic view of the operative field and tactile sensation, leading to potential inaccurate localization of a colonic lesion and difficulties with vessel ligation and lymph node dissection ( 8 ). Only a few studies have analysed the vascular anatomy of the colon using multidetector CT ( 9 10 11 ) and only one used CT colonography ( 12 ).…”
