“…Hydrocortisone displayed increased corticoid activity under occlusion for treatment of psoriasis (Sulzberger and Witten, 1961 (Feldmann and Maibach, 1965). The experimental group used plastic wrap as the occlusive agent, and had signifi cantly higher absorption of hydrocortisone than the normal, unoccluded skin.…”

“…Maximum hydration results in a 10-fold increase in penetration. This correlates with the huge increase in biologic activity of topical steroids seen with occlusive wrap in vasoconstriction assays [6] and clinical observations [7], Table IV illustrates a very important point in interpretation of the difference in biologic activity of analogs of steroids such as fluocinolone alcohol versus fluocinolone acetonide, betamethasone alcohol versus betamethasone valerate and fluocinolone acetonide versus fluocinolone acetonide 21-acetate. The biologic activity of betamethasone 17-valerate is 100 times that of betamethasone alcohol by vasoconstrictor assay [8] and superior in clinical activity in skin diseases.…”

“…Maibach [4] has shown that hydrocortisone is absorbed in tenfold greater amounts under plastic occlusion when compared to unoccluded. Sultzberger and Witten [6] showed that triamcinolone acetonide ointment under plastic occlusion was clinically as effective in obstinate psoriasis as an intralesional injection of the steroid. They attributed the enhanced activity to better contact between the ointment and the skin, more accurate localization of the ointment and increased percutaneous absorption as a result of epidermal maceration.…”
