Introduction The aim of this study was to evaluate the evidence for the efficacy of acupuncture in the treatment of anxiety and anxiety disorders by systematic review of the relevant research. Methods Searches of the major biomedical databases (MEDLINE, EMBASE, ClNAHL, PsycINFO, Cochrane Library) were conducted between February and July 2004. Specialist complementary medicine databases were also searched and efforts made to identify unpublished research. No language restrictions were imposed and translations were obtained where necessary. Study methodology was appraised and clinical commentaries obtained for studies reporting clinical outcomes. Results Twelve controlled trials were located, of which 10 were randomised controlled trials (RCTs). Four RCTs focused on acupuncture in generalised anxiety disorder or anxiety neurosis, while six focused on anxiety in the perioperative period. No studies were located on the use of acupuncture specifically for panic disorder, phobias or obsessive-compulsive disorder. In generalised anxiety disorder or anxiety neurosis, it is difficult to interpret the findings of the studies of acupuncture because of the range of interventions against which acupuncture was compared. All trials reported positive findings but the reports lacked many basic methodological details. Reporting of the studies of perioperative anxiety was generally better and the initial indications are that acupuncture, specifically auricular acupuncture, is more effective than acupuncture at sham points and may be as effective as drug therapy in this situation. The results were, however, based on subjective measures and blinding could not be guaranteed. Conclusions Positive findings are reported for acupuncture in the treatment of generalised anxiety disorder or anxiety neurosis but there is currently insufficient research evidence for firm conclusions to be drawn. No trials of acupuncture for other anxiety disorders were located. There is some limited evidence in favour of auricular acupuncture in perioperative anxiety. Overall, the promising findings indicate that further research is warranted in the form of well designed, adequately powered studies.

Aplicabilidad de la auriculoterapia para reducir estrés y como estrategia de coping en profesionales de enfermeríaObjetivos: Ensayo clínico hecho aleatorio objetivó evaluar la auriculoterapia para disminución de niveles de estrés en 75 profesionales de Enfermería de un hospital y analizar los principales dominios de Coping que se modificaron después del tratamiento. Metodología: los sujetos fueron divididos en grupos (Control, Agujas, Semillas), recibieron 8 sesiones en los puntos Shenmen, Riñón y Tronco Cerebral. El Grupo Control no recibió ninguna intervención.Resultados: se constataron diferencias significativas por la ANOVA para niveles de estrés entre el Grupo Aguja/Controle en la tercera y cuarta evaluaciones, según scores de estrés de la Lista de Síntoma de Estrés de Vasconcelos. Para el Inventario de Folkman y Lazarus, se logró diferencia estadística para el dominio Alejamiento entre los Grupos Aguja/Controle después del tratamiento; y en el análisis dentro del mismo grupo, se lograron diferencias significativas para el dominio Confrontación en la cuarta evaluación entre Grupos Aguja/Controle y para Soporte Social, en la tercera evaluación para los Grupos Aguja/Semillas. Conclusiones: la auriculoterapia con agujas apocó los niveles de estrés significativamente y hubo disminución de la utilización de los dominios Alejamiento, Confrontación y Soporte Social para los grupos de intervención después del tratamiento, sugiriendo que la auriculoterapia tanto con agujas cuanto con semillas puede producir un impacto positivo para mejorar estrategia de Coping en el equipo de Enfermería. Más estudios, sin embargo, se hacen necesarios para que se pueda conocer el alcance de la técnica.

The study was to explore whether auricular acupressure (AA) can relieve anxiety during the period from trans-vaginal oocyte retrieval to the embryo transfer in IVF treatment and whether AA can improve the outcomes of IVF. 305 infertile patients with tubal blockage who were referred for IVF were included. The women were randomized into a control group with 102 cases, a Sham-AA group with 102 cases and an AA group with 101 cases. The anxiety levels were rated with Spielberger's State Trait Anxiety Inventory and the Amsterdam Preoperative Anxiety and Information Scale. Data of clinical pregnancy rate (CPR), implantation rate (IR) and live birth rate (LBR) were obtained. The levels of neuropeptide Y (NPY) and transforming growth factor alpha (TGF-alpha) in the follicular fluids were detected with ELISA. After treatment, in AA group, the levels of state anxiety, preoperative anxiety and need-for-information were significantly lower, whereas CPR, IR, LBR and NPY levels in the follicular fluids were markedly higher than Sham-AA group and control group. We concluded that AA could help to reduce anxiety levels associated with IVF and improves the outcomes of IVF partly through increasing the levels of NPY in the follicular fluids.
