“…Not long after being first isolated in 1956, amphotericin B was found to be effective for the treatment of blastomycosis. 70 Five large series of patients showed that therapy with a cumulative dose of at least 1 g of amphotericin B deoxycholate resulted in cure without relapse in 77 to 91% of patients. 2,[71][72][73][74] Abernathy showed in 1967 that cumulative doses of 2 g resulted in cure rates up to 97%.…”

“…As reviewed elsewhere in this issue, 24 many studies, but not all, 42 found outdoor occupations made up a minority of blastomycosis cases. There have been reports of blastomycosis cases in autoworkers based on short case series or case studies, 37,[43][44][45] but no systematic investigation of this connection. Whether auto-related businesses afford opportunities for workplace exposure to Blastomyces, or whether this association simply reflects persons who disproportionately encounter the fungus during home or recreational activities, is unclear.…”

“…In contrast, B. dermatitidis rarely involves isolated skeletal muscle. In our review of over 1400 blastomycosis cases published in English (Table 1), which included cases reported before the availability of effective antifungal chemotherapy, 8 eight patients with infection of the psoas muscles and two patients with disseminated disease and unspecified skeletal muscle infection were identified 5–7,9–23 . In all cases with sufficient clinical information to assess the involvement of adjacent bone or soft tissue, psoas muscle abscesses were uniformly associated with vertebral osteomyelitis.…”
