Abstract: Spondyloarthritis (SpA) is a subset of seronegative rheumatic-related autoimmune diseases that consist of ankylosing spondylitis (AS), psoriatic spondylitis (PsA), reactive spondylitis (re-SpA), inflammatory bowel disease (IBD)-associated spondylitis, and unclassifiable spondylitis. These subsets share clinical phenotypes such as joint inflammation and extra-articular manifestations (uveitis, IBD, and psoriasis [Ps]). Inflammation at the enthesis, where ligaments and tendons attach to bones, characterizes and … Show more

“…However, the expression in ileum was not signi cantly inhibited by anti-IL-23 antibody, contrary to our expectations. The IL-23/IL-17 axis was shown to play important role in the pathogenesis of SpA through genetic, experimental and clinical studies [29]. IL-23 stimulates various cells to produce IL-17, which contributes to in ammation by increasing the production of in ammatory cytokines such as IL-6 and TNF-α.…”

“…IL-17Ai could improve peripheral arthritis early after switching than TNFi (no significant difference) even though CRP were deteriorated. It seems paradoxical, but IL-17Ai were reported to improve the enthesitis, which is one of the main pathological conditions of PsA and more strongly caused by IL-17A than TNF-alpha, 28 accordingly it was possibility that joint symptoms were improved from early after switching. It is no wonder that IL-6 receptor inhibitors, which strongly suppress inflammation, have been shown to be ineffective against PsA and other SpA.…”
