Glaucoma is a group of diseases involving the optic nerve and associated structures, which is characterized by progressive visual field loss and typical changes of the optic nerve head (ONH). The only known treatment of the disease is reduction of intraocular pressure (IOP), which has been shown to reduce glaucoma progression in a variety of large-scale clinical trials. Nowadays, a relatively wide array of topical antiglaucoma drugs is available, including prostaglandin analogues, carbonic anhydrase inhibitors, beta-receptor antagonists, adrenergic agonists, and parasympathomimetics. In clinical routine, this allows for individualized treatment taking risk factors, efficacy, and safety into account. A major challenge is related to adherence to therapy. Sustained release devices may help minimize this problem but are not yet available for clinical routine use. Another hope arises from non-IOP-related treatment concepts. In recent years, much knowledge has been gained regarding the molecular mechanisms that underlie the disease process in glaucoma. This also strengthens the hope that glaucoma therapy beyond IOP lowering will become available. Implementing this concept with clinical trials remains, however, a challenge.

Functional analysis of differentially expressed genes associated with glaucoma from DNA microarray data

ABSTRACT. Microarray data of astrocytes extracted from the optic nerves of donors with and without glaucoma were analyzed to screen for differentially expressed genes (DEGs). Functional exploration with bioinformatic tools was then used to understand the roles of the identified DEGs in glaucoma. Microarray data were downloaded from the Gene Expression Omnibus (GEO) database, which contains 13 astrocyte samples, 6 from healthy subjects and 7 from patients suffering from glaucoma. Data were pre-processed, and DEGs were screened out using R software packages. Interactions between DEGs were identified, and networks were built using Search Tool for the Retrieval of Interacting Genes/Proteins (STRING). GENECODIS was utilized for the functional analysis of the DEGs, and GOTM was used for module division, for which functional annotation was conducted with the Database for Annotation, Visualization, and Integrated Discovery (DAVID). A total of 371 DEGs were identified between glaucoma-associated samples and normal samples. Three modules included in the PPID database were generated with 11, 12, and 2 significant functional annotations, including immune system processes, inflammatory responses, and synaptic vesicle endocytosis, respectively. We found that the most significantly enriched functions for each module were associated with immune function. Several genes that play interesting roles in the development of glaucoma are described; these genes may be potential biomarkers for glaucoma diagnosis or treatment.

Резюме. Цель -изучить продукцию матриксных металлопротеиназы-2 и -9 в слезной жидкости и полиморфизм гена СFH в популяции больных первичной открытоугольной глаукомой.Исследование проведено на 120 глазах больных первичной открытоугольной глаукомой (ПОУГ), из них 34 глаза с начальной стадией, 35 глаз -с развитой, 35 -с далекозашедшей стадией и 16 глаз -с терминальной стадией. Контролем служили 25 пациентов без глаукомы. Объектом биохимического исследования служила слезная жидкость. Уровни ММП-2 и ММП-9 определяли «сэндвич»-методом твердофазного иммуноферментного анализа на обоих глазах.Материалом генетического исследования служила венозная кровь. Анализу подвергали геномную ДНК человека, выделенную из лейкоцитов цельной крови. Применяли метод ПЦР.Результаты исследования анализировали с помощью пакета прикладных статистических программ SAS (Statistical Analysis System, SAS Institute Inc., США) с применением стандартных алгоритмов ва-риационной статистики, включая корреляционный анализ и анализ таблиц сопряженности, а так-же различные типы межгруппового сравнения распределений изучаемых показателей. Установлено, что среднегрупповые показатели металлопротеиназ-2 и -9 увеличиваются по мере прогрессирования заболевания. Проведен корреляционный анализ сопряженности связи между морфометрическими, клинико-функциональными параметрами и количественными показателями ММП-2 и ММП-9. Из-учена распространенность полиморфизма гена CFH с учетом стадии развития глаукомы.Исследование представило доказательства взаимосвязи продукции ММП-2 и ММП-9 с полимор-физмом гена СFH, а также расширило список генетических мутаций, принимающих возможное уча-стие в патогенезе ПОУГ.
