The reliability of glomerular filtration rate measured from plasma clearance: a multi-centre study of 1,878 healthy potential renal transplant donors

Abstract: The CV of ECFV/BSA in normal subjects is a useful indicator of the technical robustness with which GFR is measured and, in this study, indicated a wide variation in departmental performance.

“…Using the gold standard technique of steady-state inulin clearance, the lowest CVs for GFR, in contrast, have been approximately 10% [14]. In a recent 15-centre audit focussing on interdepartmental technical performance in the measurement of GFR in healthy kidney transplant donors [15], the CVs of ECV/ BSA and GFR/BSA varied between 8-26 and 13-23%, respectively. The values in the current study based on multisample clearance are towards the lower ends of these ranges, while, with respect to ECV/BSA, the values in the current study for three-sample clearance are towards the upper end of the range.…”

“…Some work (19) has studied the reliability of GFR measurements in a healthy group of patients (no third spaces) by examining the coefficient of variation of scaled GFR measurements. The coefficient of variation of GFR/BSA was shown to be between 10% and 25% for the participating centers.…”

“…Chemotherapy doses are hence based on the patient's GFR results. The GFR-based Calvert formula (37), which is commonly used clinically for chemotherapy dosing, is as follows: 19 where dose is the carboplatin pharmaceutical dose in milligrams. The target AUC is the desired carboplatin clearance in mg/mL/min, and GFR is in mL/min.…”

“…The data on which this was based were limited, and there is variation between UK centres in the measurement of GFR. 86 In most transplant programmes living kidney donors are required to be over 18 years of age, 18 but cases where adolescents have donated do exist. 87 Caution is recommended when accepting donors under 25 years of age as younger people have more time to develop comorbidities and an increased lifetime risk of renal failure.…”
