The Red Wine Polyphenol Resveratrol Shows Promising Potential for the Treatment of Nucleus Pulposus–Mediated Pain In Vitro and In Vivo

Abstract: Resveratrol was able to reduce levels of proinflammatory cytokines in vitro and showed analgetic potential in vivo. A decrease in proinflammatory cytokines may possibly be the underlying mechanism of pain reduction observed in vivo. Resveratrol seems to have considerable potential for the treatment of NP-mediated pain and may thus be an alternative to other currently discussed (biological) treatment options.

“…Resveratrol, a naturally occurring polyphenol present in red wine, was able to reduce IL-6, IL-8, MMP1, MMP3 and MMP13 expression when injected into the IVD [170]. Rhein (4,5-dihydroxyanthraquinone-2-carboxylic acid), an anthraquinone molecule derived from the rhizome of Rheum palmatum that exhibits anti-inflammatory activity and is used in the treatment of osteoarthritis and pain relief, was hypothesized to be a therapeutic agent for IVD through the regulation of IL-1 activity [171].…”

“…The gram ratings for von Frey hairs were based on the ratings supplied by the manufacturer. The filaments were sequentially applied to the paw surface just until the filament bent and was held for 3 s. The response was considered positive if the rat lifted the foot in combination with either licking or shaking of the foot as an escape response (Kobayashi et al, 2011;Wuertz et al, 2011).…”

“…Animals from the NP application group were divided into four groups (n = 12 in each): NP + EGCG 10 µM (0.1 mL of 10 µM EGCG), NP + EGCG 100 µM (0.1 mL of 100 µM EGCG), NP + water (0.1 mL of water as a vehicle), and NP (non-treatment) group. Animals in the treatment groups were injected with 0.1 mL of the designed treatment solution into the underlayer of epineurium, just distal to the NP, before closing the incisions (Kobayashi et al, 2011;Wuertz et al, 2011). Sensitivity to non-noxious mechanical stimuli was tested using the von Frey Filament test.…”

“…Proinflammatory cytokines, such as tumor necrosis factor-alpha (TNF-a), Interleukin-1-beta (IL-1b), Interleukin-6 (IL-6), and nitric oxide, phospholipase A2, and serotonin, are known chemical inducers of sciatica [1,3,13,14,17,18,22,32]. Nucleus pulposus contains TNF-a, and nucleus pulposus applied to the dorsal root ganglion (DRG) induces pain-related behavior in rats and causes morphological and functional changes in DRG and spinal cord [13,14,26,29,30,32,37,48].…”
